Our home health care agency is rapidly expanding and we are looking to hire Home Health Nurses effective immediately. We work with patients of all ages and backgrounds who require daily care in their homes. We hire the most compassionate and thoughtful talent in the field. You will be responsible for managing a heavy case load, so time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. Home Health Nurse responsibilities:

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks, including vital signs and medication review, Oasis assessment, wound care and IV therapy.
• Dress or redress open wounds and assess progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests

To be qualified for Home Health Care:

• Active RN License in the state of practice
• Valid Driver License
• Valid Auto Insurance
• COVID Vaccine (Exemption letter may be applicable)
• CPR or BLS Certification
• Valid 2 Step TB Test (Skin test within the last year or x-ray within the last five years)
• 2 years of home health case manager experience preferred
  
• Kinnser EMR experience preferred
• availability to cover the San Antonio Metro Area and 60 mile radius from central office

This is care in a patient's home or facility of which they reside. Pay rates are established on a Tier's basis; additional compensation may be offered on a patient-by-patient basis outside of your territory.
