Legacy is recruiting an Outpatient Support Nurse Practitioner or Physician Assisan an Outpatient Advanced Practitioner for our West Linn clinic, located at 2020 8th Ave, West Linn, Oregon. Providers in this clinic include physicians and advanced practitioners. Our primary care providers are supported by individually assigned MAs as well as care teams that can include panel coordinators, social workers, RN case managers, behavioral health clinicians and pharmacy.

This is a Supportive Advanced Practitioner role. LMG Primary Care utilizes Advanced Practice Practitioners in this role to extend a group of primary care providers capacity and improve access by:

Responsibilities:

• Helping to manage the Epic In-basket (patient messages, medication management, diagnostic results and orders, and responding to urgent issues)
• Seeing routine follow-up patients which improves access and decompresses the schedules of other providers, allowing them to see more new patients
• Seeing same day urgent care type issues which mitigates disruption to the daily schedule, improves access and patient satisfaction

• Requires experience in direct patient management in a Primary Care setting.
• Knowledge of professional medical practice and care to provide/ evaluate patient care. Requires skill in applying and modifying the principles, methods and techniques of medicine to provide on-going patient care.
• Excellent communication skills required (both verbal and written).
• Basic computer skills proficiency required to navigate electronic systems applicable to job functions.
• Knowledge of acute and chronic disease states, administration and prescription of medications, and delivery of physical care following treatment plan.

• Graduate of Nurse Practitioner or Physician Assistant program.
• Minimum of 2-years’ experience.
• Current Oregon PA or NP license w/Prescriptive Privileges.
• Current BLS certification from the American Heart Association
