Job Title: Physical Therapist - Home Health
Job Type: 13-26 Week Contract
Shift: Day, 5x8-hour (08:00 - 16:30)

Location: Tallahassee, Florida 32308
Job Description:
We are looking for a licensed Physical Therapist with previous adult home health experience to join our team. The successful candidate will be responsible for providing home-based rehabilitation services to our clients, including assessments, developing treatment plans, and executing interventions to help patients regain their mobility and independence.
Responsibilities:
- Conduct in-home evaluations and assessments to determine the extent of the patient's impairment
- Develop treatment plans and intervention strategies to meet patient's individual needs
- Educate patients and caregivers on proper exercise techniques and home safety measures
- Record patient progress and adjust treatment plans as necessary to achieve desired outcomes
- Maintain accurate patient records in Home Care Home Base and Oasis systems
- Maintain productivity points of 30 or more per week
Requirements:
- Must have a state license as a Physical Therapist
- Previous adult home health experience
- Familiarity with Home Care Home Base and Oasis systems
- Ability to work independently and manage a caseload effectively
- Reliable transportation with mileage reimbursement based on IRS standards
- Willingness to work a 5x8-hour shift during the day
- Ability to communicate effectively with patients, caregivers, and other healthcare professionals
- COVID-19 vaccination required (with medical/religious exemptions only)
