Summary of Position



The Perinatal Outreach Professional will focus outreach efforts to providers of medical care and treatment, targeting pregnant people experiencing substance use disorder (SUD) as wells as those seeking medication assisted treatment (MAT) and/or are currently pregnant and already receiving MAT. This professional will act as the liaison between BCCS, client, and community medical settings intended to increase access to treatment services, transportation to OBGYN appointments, reduce stigma associated with substance use disorders while pregnant, and increase education and access to supportive services across providers within all 3 counties in the State of Delaware and other assigned duties.

The Perinatal Outreach Professional will target all Federally Qualified Health Centers (FQHC) in Delaware, including all physical locations of the Henrietta Johnson Medical Center, Westside Family Health, and La Red Health Center in all three counties of Delaware. Perinatal Outreach will also target Delaware’s Community Health Centers, including all the “satellite” Locations of Alfred I. DuPont Hospital for Children, Claymont Family Health Services, and the satellite locations of St. Francis Family Practice, including the St. Clare medical van and their Center of Hope. Perinatal Outreach efforts will also focus on emergency departments in hospitals statewide, private practice OB/GYN groups, community-based organizations (particularly those with early childcare/early child development programs ex: DAPI) and interested faith- and community-based institutions.

Qualifications

REQUIRED: Associates Degree
PREFERRED: Bachelor's Degree
REQUIRED: 1 year of intensive case management services experience
PREFERRED: CADC or CADC eligible

Schedule
Monday-Friday 7am-3pm

Starting at $17.hr, commensurate experience
