The Perinatal Care Coordination Specialist, Housing Programs, is accountable directly to the Perinatal Services Program Manager; Works as part of an integrated care team to improve maternal and child health outcomes for families receiving services from COH housing programs by ensuring access to timely perinatal health services; Educates participants and staff on issues of perinatal health to support families during pregnancy and infancy; particularly for families who do not receive care or care coordination services from COH. This is a full-time position.

Highlighted Duties and Responsibilities:
  • Supports and closely coordinates perinatal care coordination with the participant’s housing support team and, when appropriate, other maternal child health providers.
  • Provides care coordination services to a case load of approximately 40 participants to ensure access to high quality perinatal health and early child health services.
  • Conducts thorough assessment on all participating prenatal/postpartum participants utilizing OB Authorization form, current health screening tools (HITS, 4P’s, PHQ-9, Edinburgh, etc.).
  • In collaboration with the housing case manager, develops individualized care plans that address risk factors, including but not limited to social needs, barriers to care, reproductive life planning, and food insecurity.
  • Completes home visits and/or telephone outreach biweekly, and accompanies participants to Obstetrician visits if necessary, to ensure connection to care.
  • Provides health and parenting education to participants in a variety of settings, with an emphasis on increasing understanding of healthy pregnancy and warning signs, nutrition, self-management of care and primary care.
  • Recruits program participants from COH’s housing programs and meets initially with at least 80% of eligible clients
  • Facilitates biweekly communication with housing case managers, through shared case notes, emails, and in-person case reviews as needed, to ensure effective coordination of care between perinatal and housing services.
  • Participates in housing program meetings quarterly and as needed, by site, to share successes, challenges, and changes to the perinatal care referral and intervention process.
  • Completes and locks all progress notes within 3 business days after participant visit.
  • Documents all participant encounters, including face to face, telephone, and electronic communications, in the medical record within 2-3 business days.
Minimum Qualifications:
  • B.A./B.S. in social work or education or equivalent experience required.
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date
  • Experience providing social services, care coordination or health care education required.
  • Experience working with underserved population, including pregnant women experiencing homelessness, preferred
  • Experience in clinical setting required, preferably in community health environment
  • Ability to travel between COH sites and to participants homes required
  • Familiarity with Medical Assistance system and community resources strongly preferred.
  • A valid driver’s license and regular access to a car, as well as proof of auto insurance, required.
  • All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines. Medical or religious exemptions may be requested.
