Looking for a motivated and hardworking candidate to join our team. Ideal candidate would have previous experience working in an assisted living setting performing patient monitoring activities. A passion for the elderly population is a must!
Responsibilities Include but are not limited to:
- Measure, collect and document patient rhythms by using devices to measure heartrate, blood-oxygen levels, blood pressure and others as related to the patient’s care plan
- Monitor changes in patient's health rhythms and escalate as needed
- Enroll and get consent for all eligible Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) patients
- Collect responses to screening questions and document responses
- Patient Care Coordination (Chronic Care Management)
Qualifications:
- High school diploma required. Associates degree or higher preferred
- Must demonstrate a thorough understanding of the monitoring process and effective, efficient use of the monitoring equipment and system
- Previous experience in a similiar role
- Knowledge of eclinicalworks storngly preferred.
- Travel between facilities as needed
Position will cover Assisted Living Facilities in the county working in an out patient provider office located inside the facility.
Schedule is 5 days per week (24-28 hours).
