Patient Care Coordinator -FULL TIME, CAN Pharmacy Jacksonville, FL

ESSENTIAL FUNCTIONS (This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):

• 
  Accurately inputs new referrals into the computer system and updates information in the computer system (Asembia, Excel, GRx, etc).
• Communicates to the stakeholders the necessary information to obtain benefit information, authorization, reauthorization, and reimbursement by emailing and scanning pertinent medical information.
• Answers new referral intake and/or refill calls in a timely and professional manner.
• Contacts patient to collect information necessary to establish his/her medical chart. Discuss financial obligations with the new patient.
• Addresses insurance issues that require alternate funding. Registers and/or opens patients with manufacturer reimbursement programs, as needed.
• Follows-up on authorizations to ensure they are achieved prior to dispensing.
• Coordinates with referrals source all necessary information needed to initiate services for new patient referrals.
• Schedules dispensing calendar for new or refill customers.
• Sets up shipping arrangements as needed.
• Participates in the Performance Improvement Process.
• Assists in co-pay collections, which require communication with the patient.
• Reviews dispensing calendar and assists with the completion of all order entries and shipments each day.
• Enters new referral prescriptions accurately into computer and has Pharmacist verify correctness of the prescription entry.
• Gathers medications and supplies, labels, and has shipments verified for correctness by the Pharmacist.
• Assists the dispense technicians as needed regarding required refill requests that are discovered at time of dispensing.
• Other duties as assigned

QUALIFICATIONS: Current, Unrestricted FL Pharmacy Technician license

Education: High School Diploma or equivalency. National and/or State Certified Pharmacy Technician license, depending on requirements by location.

Experience: Previous pharmacy experience and customer service experience a MUST

Knowledge, Skills, and Abilities:

• Working knowledge of computer software, specifically Microsoft Word
• Ability to accurately enter data into computer
