CLCH are looking for an enthusiastic and self-motivated Nutritionist to join our Wandsworth Healthy Lifestyle Team.

The Wandsworth Healthy Lifestyle Team work across the borough deliver Health4Life weight management programs to children aged 2-11 years as well as Mothers with a baby under 2 years old. The team work alongside the Health Visitors and School Nurses to provide excellent locally based care to families within the Wandsworth borough.

You will need to display excellent communication skills and be able to work in a kind and compassionate manner to deliver motivating Nutrition Education sessions. You will need to be creative in your approach in order to engage children of all ages. Mobile and agile working is a key part of the way the services operates so you will need to be comfortable travelling between venues throughout the week.
  • To work independently under the supervision of Nutritionist Lead to undertake clinical tasks to support the development and delivery of the healthy weight programme across Wandsworth
  • To establish, organise and co-ordinate all systems relating to the administration, advertising and delivery of the childhood healthy lifestyle programmes
  • To be responsible for organising own delegated clinical work load and administrative tasks and activities and to be flexible to the needs of the service
  • To be responsible for inputting clinical activity data into the relevant IT system and generate reports as required on a regular basis
  • To work within an agreed framework for dealing with routine and urgent clinical problems/interactions, face to face or on the telephone, seeking guidance from the Nutritionist/Infant Feeding Lead to agree action outside the framework
  • To communicate in an effective and timely way with accurate message taking from patients, carers and other healthcare professionals with clear record keeping and dissemination of information to relevant staff
  • To facilitate parent and childhood healthy lifestyle group education programmes
  • To support in delivery of training activities, as required
  • To support the Nutritionist Lead in the future developments and co-ordination of a variety of Healthy Weight work stream initiatives
  • To participate in clinical governance activities within the Healthy Weight Service

  • Communicating with patients, carers and health care professionals to ensure nutritional requirements are assessed and addressed
  • To be responsible for undertaking basic clinical investigations following agreed protocols such as height, weight, calculating BMI and BMIz score, body composition and waist circumference
  • To record patient’s anthropmetric measurements and other relevant details using standard recording procedure and adhering to department standards
  • To assist in the delivery and evaluation of a family-based healthy lifestyle programme
  • To be responsible for the marketing and recruitment of participants onto intervention and training
  • To review clients compliance with nutritional advice previously given and goal achievement.
  • To provide appropriate first line advice as agreed with line manager to patients, carers, medical staff and other health care professionals
  • To support patients in accessing different professionals and agencies by listening to request or issues and directing them appropriately
  • To be responsible for liaising with GPs, Practice Nurses, Health Visitors, School Nurses and other Health Care Professionals to collect all relevant medical and social information prior to nutritional intervention
  • To assist in the evaluation and collation of results from training and intervention This will include clinical data, attendance rates, did not attend rates and qualitative data to identify trends and make recommendations on service improvement
  • To be responsible for implementing Trust and departmental policies in own work stream such as clinical record keeping, patient confidentiality, lone worker policy
  • To be professionally accountable for one’s own work within an agreed framework and to ensure all work undertaken within professional capability, professional standards and clinical guidelines
  • To be responsible for complying with legal and professional confidentiality guidelines at all times
  • To facilitate childhood and post-natal healthy lifestyle group education programmes
  • To be creative and organised and ensure that group education programmers run smoothly, and activities are educational and funTo be responsible for supporting all administrative aspects of the childhood healthy lifestyle This will include reviewing the referral process, developing and maintaining the booking system, identifying and co-ordinating appropriate venues, organising programme dates, coordinating speakers, collecting together relevant resources and equipment and development of standard letters
  • To be responsible for marketing and promoting of relevant local This will include developing promotional material, meeting GPs, Practice nurses and other health professionals to discuss the programme, promoting the programme to community groups and local schools
  • To be responsible for the promotion and recruitment of participants
  • To be responsible for ensuring all clinical and non-clinical data relating to service is entered onto the appropriate IT systems
  • To be responsible for collating the results. This will include clinical data, attendance rates, did not attend rates and qualitative data
  • To be responsible for designing and compiling regular evaluation reports. This will involve ensuring the database systems are in place to collect appropriate data, writing reports, identifying trends and making recommendations to improve the service.
