The Shift Nurse is responsible for administering appropriate medications, documentation, and on-going nursing assessment of assigned person served. This position is full time and includes day, evening, and weekend hours.

Duties and Responsibilities:
1. Provide on-going nursing assessment of each assigned person served to ensure that the resident’s total health needs are met
2. Documenting and maintain accurate medical information in the individual files.
3. Administering appropriate medications at prescribed times and responsible for proper medication administration in accordance to self-administration medication program and procedures developed by the QIDP
4. Assist in developing procedures which will provide for proper and safe administration of medications.
5. Responsible for proper administration of G-tube feedings and medications via G-tube or delegating such to certified medication aide on duty if otherwise engaged.
6. Responsible for assuring that persons served have a sufficient supply of medication and supplies if they are to be away from the facility.
7. Training staff in appropriate areas as determined by the ICF/ID Manager as assigned by ICF/ID Manager of Nursing.
8. Serve as ICF/ID On-Call nursing staff on rotating basis as scheduled.
9. Be available during assigned hours to cover medical concerns, medical trips, hospital admissions, or other related health matters as necessary.
10. Maintain compliance with the Department of Inspection and Appeals (DIA) and other regulatory agencies regulations and participate in surveys.
11. Assisting with writing reports and if deemed necessary, attend a resident’s staffing.
12. Conferring with the school nurse as necessary, in order to meet individual needs and promote continuity of care.
13. Maintain legal responsible party/legal guardian contact in regard to medical issues.

Qualifications:

• Current and maintenance of Iowa L.P.N. or R.N. license
• Valid Class C driver’s license required, unless driving a vehicle with a capacity of 8 or more, then a Class “D” license will be required and be an insurable driver with our insurance company.
• Live within 30 miles driving distance from FHC preferred.
• Clearance from all applicable background checks as required by service standards.
• Computer literate or have basic knowledge of computer skills
• Ability to make sound decisions in all staff and programming matters
• Comply with ICF/ID CMS health requirements.
• Reliable Transportation.
• Provide proof of state minimum automobile insurance coverage if applicable. Be an insurable driver with our insurance company.
• Ability to work with children with mental illness, intellectual disabilities, and behavior concerns, providing support in all aspects of daily living as directed by each person’s individual plan.

Physical Requirements:

• Requires physical mobility to walk, stoop, stand, sit, and climb steps.
• Requires the physical strength to assist individuals in transferring as per Hope Haven policy.
• Requires physical strength to restrain an individual as outlined in the SCM procedures.
• Ability to drive
• Ability to lift 50 pounds.
