OBJECTIVE SUMMARY
: The Intake Coordinator manages referrals and phone calls related to Admissions to the Medical Unit. The Coordinator works with surrounding facilities to provide ease and safe transfers of those patients that have been accepted into the Swing Bed Program. The position also supports activities related to Utilization Review and ensures all patients continue to meet criteria for continued stay. This position ensures delivery of all clinicals to insurance companies as requested by the insurance company. Works in collaboration with nursing, medical, and rehabilitation departments to assure appropriate and timely referrals.


ESSENTIAL FUNCTIONS: Ability to assess, plan, implement, monitor, and evaluate incoming and current patients for admission and continued care criteria. Ability to work well with others and to coordinate patient care needs.

  • Reviews all intake referral documents utilizing nursing assessment and clinical skills for suitability of admission
  • Consults with Manager and other disciplines on medical suitability of the referral
  • Assures that all documentation such as discharge orders, treatment care plan, discharge plan and other relevant clinical information is reviewed and complete before accepting patient
  • Reviews the insurance verification per hospital policy before accepting patient
  • Participates in the daily huddles, treatment team meetings as requested
  • Works in a collaborative manner with other team members to assure daily coverage for admissions and referrals
  • Participates in Care Conferences with other team members and /or family as requested
  • Maintains thorough documentation and reporting to appropriate county, state and federal entities
  • Participates in hospital quality initiatives and strategic planning activities
  • Maintains data and tracking metrics as established within the department and hospital
  • Participates in ongoing chart review and assessment of the patient population served to assure appropriate services are available


REQUIRED EDUCATION: Registered Nurse, Bachelor’s Degree preferred

PREFERRED EDUCATION: Case Management and or Utilization Review Certification

REQUIRED EXPERIENCE: Minimum one year of previous experience in a hospital or institution

PREFERRED EXPERIENCE: Five years nursing experience

ADDITIONAL ELIGIBILITY QUALIFICATIONS: Utilization Review

WORK ENVIRONMENT: Shared office space with other hospital department staff members.

PHYSICAL DEMANDS: Ability to maintain focus while working in a busy office environment.


OTHER DUTIES:

  • Will assist in all assigned areas of Swing Bed Intake Referral Processing
  • Will assist in Utilization Review Tasks
  • Ability to self-start and to prioritize daily tasks
  • Multidisciplinary Care Plan Meeting
  • Will assist with Administrative duties as assigned.

Eligibility Requirements - Employees regularly scheduled at least 20 hours per week.
