POSITION SUMMARY:

This position includes nurse case management duties for patients with substance use disorders (SUDs) and for patient in the Medication Assisted Treatment (MAT) Program. The incumbent 1) collaborates with Chapa-De providers, other staff members and community partners to improve treatment outcomes for patients with SUDs and 2) assists the Behavioral Health Director with MAT program development and workflows and compliance with grant management and reporting requirements.

DUTIES AND RESPONSIBILITIES:
Provide case management and collaboration with Chapa-De providers and community partners to improve treatment outcomes for patients with SUDs. This includes:

  • Overseeing the Screening, Brief Intervention and Referral to Treatment (SBIRT) program at the Chapa-De Indian Health Program
  • Assessing patients’ SUD level of care needs via ASAM criteria, suitability for medication assisted treatment and behavioral health counseling needs and preferences
  • Referring patients for behavioral health treatment
  • Assisting with induction, stabilization and maintenance phases of patients’ MAT care
  • Understanding and implementing policies and procedures to ensure compliance with regulations and reporting requirements for specific programs (SBIRT) and SUD-related grants
  • Acting as the liaison to community, county, state and Indian Health Service agencies on behalf of Chapa-De patients diagnosed with SUDs
  • Visiting Chapa-De patients when they are hospitalized, incarcerated or in other treatment facilities to provide continuity of care as needed

Experience and Skills:

QUALIFICATIONS:

  • BSN or MSN degree


PREFERRED QUALIFICATIONS:

  • Addictions nursing specialty certification.
  • Board Certified in Addiction Nursing.
  • MAT experience.
  • Experience working with SUD.
