POSITION SUMMARY:

The Licensed Marriage & Family Therapist (LMFT) or MFT is responsible for providing professional and supportive counseling, crisis intervention, solution focused treatment plans, psychosocial and mental health assessments, linkage to community resources and referrals (mental health, medical support housing, substance abuse, food etc.), mental health education, and advocacy for patients medical and mental health needs.

LMFTs are responsible for evaluation, diagnosis, and treat mental and emotional disorders, health issues, and behavioral issues, and a wide range of relationship dynamics within the context of couples, marriage, and family systems.

Marriage and family therapists are responsible for a wide array of serious clinical problems, which include but are not limited to: depression, anxiety, marital issues, individual psychological problems, and complications surrounding the parent-child relationship.

PRIMARY RESPONSIBILITIES:

● Work with adult patients with primary diagnosis of medical disorders in outpatient medical settings

● Document clinical notes using computer software program (electronic health records)

● Conduct case management

● Responsible for facilitating and coordinating behavioral services for enrolled ECM members

● Connect member to other social services and supports they may need

● Coordinate with hospital staff on discharge plan and other transitional care needs

● Engage with enrolled members

● Complete comprehensive assessment, develop and update member’s care plan.

● Offer services where the member lives, seeks care, or finds most easily accessible and within managed care plan (MCP) guidelines. This includes home visits.

● Advocate on behalf of members with healthcare professionals

● Use motivational interviewing, trauma- informed care, and harm-reduction approaches

● Accompany member to office visits, as needed and according to MCP guidelines

● Monitor treatment adherence

● Provide health promotion and self- management training

● Act as clinical resource for care team

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

● Demonstrate ability to perform assessments and supportive counseling as it pertains to medical and behavioral health support

● Demonstrate cultural sensitivity & competence in addressing the needs of diverse patients

● Possess excellent communication and writing skills, including ability to write clear and concise clinical case notes, documents, and reports.

● Possess excellent interpersonal skills & ability to collaborate with & respond to multiple specialties.

● Demonstrate ability to establish and maintain effective working relationships.

● Demonstrate ability to make independent decisions, exercise sound clinical judgment.

QUALIFICATIONS:

  • Education: Master's degree in Marriage & Family Therapy or related field (Required).
  • Experience: Minimum two years of experience as a licensed LMFT preferred.
  • New graduates highly encouraged to apply.
  • MFT License preferred.

All patients are subjected to COVID screening and are required to wear a mask when not receiving treatment. All staff members must be fully vaccinated.

Job Type: Full-time

Pay: $30.00 - $31.00 per hour
