JOB SUMMARY:

Under general supervision, this position functions as a provider of school based brief mental health counseling and early interventions services. This position is responsible for providing group and rehabilitative prevention services, for children and youth who have been impacted by adverse childhood experiences, have social-emotional or behavioral issues, and/or at risk of severe emotional disturbance. Services may be provided in the school, or in the classroom but may be extended into the home, which will increase the child’s ability to learn and develop. This position will collaborate with Mental Health Clinicians to complete comprehensive assessments and deliver psycho-educational student groups.

MINIMUM REQUIREMENTS / QUALIFICATIONS:

Education:

Graduation from an accredited four-year college or university with major course in social services, Psychology, Child Development or a closely related field.

Experience:

At least 2 years of relevant experience working with children and families or social work case management.

Description of Duties:

1. Represent the agency in a consistently professional manner, always modeling a commitment to the agency’s Mission and Values.

2. Maintain case files and data base programs on a daily basis and assisting with any related statistical and/or evaluation components.

3. Provide intense home-based services (as defined in the Outpatient Contract) to assigned youth and families; focusing on development of youth and family needs, access to various resources, and overall support with mental health issues challenging the family and/or identified client.

4. Assume responsibility for participating in client assessments, implementation of client plans, and progress notes for each client.

5. Complete visits independently in the home or community to meet with clients and families.

6. Must be able to utilize appropriate field safety judgement and address concerns/challenges/barriers with supervisor.

7. Conduct risk assessments and follow safety protocols consistent with CAPC standards.

8. Maintain regular communication and collaboration with mental health clinician(s) & other school staff to ensure supportive and multi-disciplinarian services are provided to clients and families.

9. Have a solid working knowledge of all the rules, regulations, and laws that apply to the contracts.

10. Have a solid working knowledge of behavior interventions and therapeutic techniques for different age groups and for different needs; seek out guidance and training as appropriate.

11. Have a basic knowledge of mental illness diagnoses.

12. Maintain billable rates of no less than 65% productivity. Demonstrate ability to appropriately use correct billing codes and note writing strategies that do not result in disallowances during audits, etc.

13. Demonstrate proficiency in utilizing the San Joaquin County assigned electronic medical health records database, Clinicians Gateway, independently.

14. Establish trust with families through family centered support and encouragement.

15. Use a strengths-based approach and perspective at all times, while implementing motivational interviewing techniques and knowledge of protective factors in work.

16. Coach parents to act as advocates for themselves and their children to provide a healthy/safe environment for children.

17. Provide crisis intervention as appropriate and complete child abuse reports consistent with mandated reporting laws.

18. Participate in all relevant meetings for client such as: IEP, CFT meetings to ensure collaborative effort for coordination of client services.

19. Develop and maintain healthy working relationships with practitioners involved with client including social workers, school personnel, etc.

20. Assist the Clinical Director and/or Program Manager as requested.

21. Promote and maintain a sense of teamwork between CAPC staff, community partner agencies, and other individuals relevant to the successful case management of each case.

22. Network with other community agencies and participate when requested, in community and health fairs and other events designed to promote CAPC programs.

23. Other duties, both program specific and CAPC related, as assigned.
