As a MDS Coordinator, you will be responsible for assessing residents physical and mental functions, documenting data on a minimum data set form, determining appropiate referrals to ther health care professionals and using the resident assessment protocols to implement and oversee the resident care plan process.

MDS Coordinator day-to-day will include:

• Leading or assisting in leading resident care plan meetings and following up on issues with appropriate staff
• Completing all annual, quarterly and significant changes MDS assessments
• Continuously updating care plans according to any changes in resident condition, new medications, weight losses, hospitalizations and behavioral changes
• Maintaining knowledge of policy and procedures related to care plans, MDS and restraint protocols
• Reviewing care plans for appropriate goals, problems, approaches and revisions based on nursing needselps residents maintain independence, promotes dignity and physical safety of each resident.
• Assisting the DON with monitoring the review of nurse’s notes to determine if care plans are being followedractices good standard care precautions of cleanliness, hygiene and health.
• Conducting daily nursing unit observations for quality assurance, compliance and education purposes

What we are looking for:

We are looking for part time MDS Coordinator for day/evening shift.

• Strong interpersonal skills, especially when it comes to delivering sensitive information to patients and/or families.
• Ability to think critically under pressure.
• Must have and maintain a current, valid RN/LPN license.
• Possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
• Be able to make independent decisions concerning above job duties.
• Possess the ability to deal tactfully with personnel, residents, family member, visitors, government agencies/personnel, vendors, and the general public.
• Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
