The MDS Coordinator, under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare & Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team.

Essential Functions and Responsibilities:
1. Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.

2. Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.

3. Safety concerns are identified, and appropriate actions are taken to maintain and

assure patient safety.

4. Communicates schedule to all other departments and families, as required.

5. Monitors clinical systems for changes in patient/resident condition.

6. Responds to patient/resident change in condition by coordinating patient/resident re-assessment and re-evaluation plan of care.

7. Strategically sets assessment reference date (ARD), in conjunction with the

interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident.

8. Accurately completes portions of MDS and CAAs as assigned, in accordance

with policies, procedures, and guidelines.

9. Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process.

10. Coordinates and develops plan of care as required.

11. Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file.

12. Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.

13. Communicates information to nursing administration, business office, and facility administration regarding assessment compliance, care plans, updates and revisions.

14. Assures competency of all staff members involved in the RAI process and

educates the interdisciplinary team.

15. Coordinates and attends daily Casemix Meetings.

16. Attends OBRA required Casemix Meetings.

17. Maintains assessments on the active clinical record for a minimum period of 15 months.

18. Electronically transmits MDS data to the State in accordance with regulation and

facility standards, policies and procedures.

19. Remains current with industry changes.

20. Educates a back-up person for the NAC position and continues to periodically. train this person as needed.

21. Travels as needed.
