The MDS Coordinator reports to the Director of Nursing, however is part of the Quality and Clinical Revenue team and is responsible for ensuring the completion of the Resident Assessment Instrument (RAI) process.

As part of a team, guidance, direction and education may be provided by others within the Quality and Clinical Revenue team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Completes accurate and timely MDSs, CAAs and care plans in accordance with the RAI guidelines for residents.
  • Completes required assessments, interviews and documentation in an accurate and timely manner
  • Communicates resident assessment information to appropriate team members.
  • Educates and communicates the residents plan of care with team members.
  • Attend and participate in department/facility meetings
  • Leverage technology to include MyUnity, Team TSI, Abaqis, CASPAR, etc. to ensure the most accurate MDS is submitted and avoid errors.
  • Coordinates the transition of care provided in the community with that care provided in the facility to ensure accurate diagnosis, medications and follow-up.
  • Assumes the role of team lead and those responsibilities when needed.
  • Adheres to facility policies and procedures and participates in facility quality improvement and safety programs
  • Promotes and demonstrates positive customer service towards VHS, co-workers, residents and visitors
  • Performs other duties as assigned.

QUALIFICATIONS AND REQUIREMENTS:

  • Graduate of an accredited School of Nursing and holds current Virginia license. RN Preferred. LPN considered with years of experience in long term care.
  • Experience/education in Gerontological Nursing required.
  • RAC-CT preferred or willing to obtain within 1 year.
  • Computer knowledge preferred.
  • Ability to read, write, analyze, and interpret information.
  • While performing the essential functions of this job, the employee is regularly required to walk, turn, bend, stoop, lift, push, pull, reach, talk, hear, taste and smell.
  • Must be able to lift, push, pull and move up to 50 pounds.
  • Risk of exposure to blood borne pathogens and hazardous materials.
