Valley View Retirement Community is currently seeking a

Lead RNAC/Care Plan Coordinator

Primary purpose of your job position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and the polices and goals of this facility. To conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations and guidelines that govern the resident assessment, including the implementation of CAA’s and Triggers. Responsibility to maintain, review and update written policies and procedures yearly that govern the development, use and implementation of the resident assessment (MDS) and care plan.

Qualifications required:

• Maintain a current RN license in the state of PA and be in good standing
• Knowledge of Point Click Care preferred
• Preferred to have two year experience as a supervisor in a hospital, nursing care facility, or other related health care facility
• Training in rehabilitative and restorative nursing practices preferred
• Must adhere to and promote the mission and values of Valley View Retirement Community.
• Must be able to read, write, speak, and understand the English language with good communication skills
