LVN Hospice Nurse– Norwalk

Job description

We are seeking an LVN Hospice Nurse to join our hospice team. The ideal candidate has experience with patients who are terminally ill and will coordinate essential doctor visits, coordinate with hospice staff and external entities, and provide an exceptional level of care to our patients. They will also help coordinate support systems for families.

Responsibilities

•The ideal candidate has experience with patients who are terminally ill and will coordinate essential doctor visits, coordinate with hospice staff and external entities, and provide an exceptional level of care to our patients

•They will also help coordinate support systems for families

•Create treatment summaries by interviewing patients, family, and caregivers and using information from treatment plans, hospital records, and other sources

•Schedule patient appointments; supervise other hospice staff; assess the effectiveness of patient care

•Document reasons for admission or discharge from hospice

•Answer questions about hospice services; refer patients to local providers

•Communicate concerns about quality of care to management

Pay:

· Market Rate

Experience Hospice:

· 1 year Hospice Experience (Required)

License/Certification:

• LVN (Required)

Experience:

· EMR/ Hospice MD proficiency preferred.

Job Types: Full-time, Part-time

Pay: From $55.00 per hour
