Job Description- We are looking for detail oriented and experienced LPN's to join our team at Prairie Bluffs! Full time and part time positions available. The LPN is responsible for ensuring medication management services are carried out according to care plan set forth by the RN. Transcribing, ordering, and auditing for medication compliance are the primary duties of this position. In addition, the LPN in conjunction with the RN delivers exceptional service by meeting client’s needs and by ensuring all health needs, the safety of all clients and personal care needs are met.

Essential Functions (Including but not limited to the following):

  • Accurately transcribes physician orders
  • Checks physician group portals daily for new orders and recommendations.
  • Proactively engages in medication renewals to ensure medications are available for administration.
  • Follows quarterly pharmacy medication reconciliation recommendations and communicates to appropriate parties.
  • Fills weekly pillboxes for medication administration.
  • Ensuring medications are destroyed per facility policy.
  • Participates in weekly auditing of medication administration records.
  • Follows up with daily medication dashboard and missing documentation.
  • Processes medication errors and follows the direction of the Director of Nursing.
  • Works with nursing team to check in monthly medication cycle and deliver to resident apartments.
  • Manges INR monitoring for residents on coumadin therapy.
  • Communicates with families in regard to medications and costs associated with medication management services.
  • Assists with compliance in meeting regulations and is available for State Licensure Surveys.
  • Participate in staff meetings, committees, care conferences, etc.
  • Assists other departments as necessary.
  • Report injuries for self, staff, clients, or visitors immediately to RN Supervisor.
  • Proactively communicate and update key leaders with crucial information related to clients and personnel.
  • Maintain a high level of confidentiality regarding all client, personnel and vendor matters.
  • Clear and concise documentation in electronic charts.
  • Inform leadership of changes in client's needs and conditions.
  • Responds to client concerns or complaints and reports them to appropriate parties.
  • Maintains an up-to-date knowledge of current trends of Assisted Living Requirements.
  • Participate in on-call management rotation. Required Qualifications
  • Ability to lift/carry objects weighing up to approximately 30 lbs. or more and to meet all physical requirements related to this position.

Preferred Qualifications

  • Licensed with the State of Minnesota as a L.P.N.
  • Experience in working with older adults and order transcription.
  • Minimum of three years of experience in a related field.
