GCHCC is seeking a Licensed Practical Nurse (LPN), with experience in post-acute and long-term care services.

Þ The Licensed Practical Nurse is responsible for delivery of resident care through the nursing process of, planning, implementation, and evaluation. Directs and guides the activities of nursing personnel while maintaining standards of professional nursing. Responsible for the passing of medications and administrating of treatments as prescribed by a physician.

Þ Provide nursing care in accordance with physicians’ orders for each resident in conformance with recognized nursing techniques and nursing judgment not to exceed Standards of Nursing Practice as set forth within the Nurse Practice Act of New York State.

Þ Administer and document medications and treatments.

Þ Maintain accurate and descriptive records of all treatments, nursing procedures, and medications administered.

Þ Maintain established inventory standards which includes accounting for controlled substances at the start and finish of each shift when administering medications.

Þ Maintain medication cart and med room in a clean and safe manner.

Þ Handle disposal of all sharps in accordance with policy and procedures.

Þ Observe, record, and monitor resident status reporting any symptoms indicative of significant change to the Registered Nurse daily. Record observations in the medical record during their shift.

Þ Participate in direct resident care as necessary.

Þ Assist with Nursing Assistant responsibilities as necessary to ensure the best possible care of residents.

Þ Assist in the admission and discharge of residents.

Þ Supervise Nursing Assistants as they provide direct resident care.

Þ Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department

Þ Must have a working knowledge of Electronic Medical Record systems (EMR)

Þ Must display integrity and professionalism

Þ Must have strong leadership skills and be able to lead a team of nursing staff, as well as help foster a team atmosphere with the nursing department

Þ Provide thorough and comprehensive report of resident status to on-coming nurse as assigned.

Þ Maintain awareness of physical environment of the unit to ensure the safety and well-being of all residents and staff.

Þ Comply with all Infection Control Procedures as it pertains to Resident care and staff.

Þ Serves as a professional role model in conduct and appearance.

Þ Assist in transcription of physician’s orders.

Þ Participate in daily documentation responsibilities of the Nursing Unit.

Þ Is knowledgeable of Corporate Compliance responsibilities and adhere to current rules, regulations, and policies that affect his/her specific job functions/responsibilities.

Þ Assist other employees and/or departments with compliance issues and demonstrates support of the Corporate Compliance program.

Þ Report compliance issues/concerns in a timely and appropriate manner.

Þ Abide, respect, and promote the Value and Mission statements of the organizations.

Þ Other duties and responsibilities, as assigned by Supervisor / Director.

Þ Must be able to teach and lead by example

Þ Liaisons with residents, families, and support departments to adequately plan for resident needs

Þ Must be knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the long-term care facility in New York State.

Education:

Graduation from an approved school of practical nursing with a current unencumbered license to practice in New York State.

Preferred Experience:

Must have at least three (3) years of successful progressive experience in a long-term care or sub-acute setting

Required Skills:

Good knowledge of nursing principles and techniques and their relation to medical surgical practices and skill in their application; and ability to secure the cooperation of others; ability to understand and carry out technical, oral, and written instructions; ability to plan and supervise the work of others; good observation; initiative, well-motivated; mental alertness; thoroughness; patience; emotional stability; empathy; physical condition commensurate with the demands of the positions. Moderate to heavy lifting, no more than 30lbs. CPR Certification required.

Essential Skills:

Ability to speak English, see, stand for long periods of time; ability to kneel and balance oneself; ability to various nursing equipment. Lift and transfer residents appropriately up to 30 pounds with proper body mechanics.

Interpersonal Contacts:

All staff, residents, family members, Physicians, and other outside contacts.

Working Conditions:

Works on nursing units, well lighted, exposure to bloodborne pathogens, pathologic disease or elements, exposure to chemical products. Required to work weekends and holidays
