Springwell is working with MCCN to facilitate the appropriate delivery of health care services and improve health outcomes!

Position Description:

The Long Term Services & Supports Care Coordinator will provide care coordination activities to members such as organizing care and facilitating communication across medical, behavioral health, LTSS, social, and pharmacy providers, agencies, and supports. The role will also navigate individual and family service systems and work collaboratively and effectively with families and community-based colleagues.

Our ideal candidate will have experience working with individuals who have complex needs in roles such as a community health worker, outreach worker, peer specialist, or recovery coach.

Qualifications:

  • Bachelor’s degree in social work, human services, nursing, psychology, sociology, or related field from an accredited college/university OR an Associate’s degree and at least one year professional experience in the field OR at least three years of relevant professional experience.
  • Care Coordination and Behavioral Health experience preferred.
  • Ability to communicate effectively verbally and in writing.
  • Strong organization skills with attention to detail and ability to multi-task
  • Must have a reliable car, current driver's license and safe driving record
