Valley Pointe Nursing & Rehabilitation Center is a beautifully renovated boutique skilled nursing facility next to Eden Hospital. We are the #1 preferrefed facility in Castro Valley and we are also ranked “Five Stars” by Medicare and named “Best Nursing Home” by U.S. News and World Report.

We are a 50 bed facility with outstanding physical, occupational and speech therapy programs to help those with post hospital care needs to transition home. Our team is made up of talented and compassionate individuals that help each other reach our respective goals on a daily basis. Our unique and fun atmosphere makes this a favorite facility among long term care professionals and our team members love showing you what makes us so special. We're looking to expand our team. We are currently hiring for the following shifts:

• Full Time - AM Shift
• Full Time - PM Shift
• Full Time - NOC Shift

JOB FUNCTIONS:

• Make daily rounds on assigned unit to observe, assess or interview residents verifying information for or from ongoing shift, monitor regulatory compliance and to determine staff assignments or completion of assignments
• Document Med Pass in the electronic MAR system (PCC), daily charting or COC for residents.
• Administer medications, treatments and provide direct care to residents on unit according to physician orders and in compliance with facility policies and procedures
• Perform physical observation of residents as indicated by change in condition or as required by regulation. Complete assigned sections of resident observation instruments used in facility.
• Coordinate care and delivery of services with all disciplines, outside agencies and community providers as required
• Respect rights of residents regarding freedom of choice, consent for care and services, refusals of treatment and implementation of advance directives if any.

REQUIRMENTS:

• Active LVN license
