As a Home Health LVN you are not an urgent care provider, you are the stepping stone between post-acute care and independent living.

Monarch is looking for LVNs to cover:

• East County: San Carlos, La Mesa, El Cajon, Lakeside
• South East County: Rancho San Diego, Spring Valley, Lemon Grove
• South Bay: Bonita, Chula Vista, Otay Lakes, Imperial Beach, National City, and San Ysidro
• Coastal: Point Loma, Ocean Beach, Pacific Beach, Coastal Inland
• Downtown, North Park, South Park, College, Mid City

To excel in this environment, the following skill sets are necessary:

• A desire to work with patients alongside their families
• Ability to work independently to provide skilled, individualized care for patients in their homes.
• Implement therapeutic, safe treatment plans, and to provide teaching for the safest and most efficient care of patients in their homes.
• Communicates with Physician(s) and Clinical Supervisor(s) regarding patient care
• Demonstrates complex problem solving and the use of critical thinking skills
• At least one year of clinical experience
• Experience with wound care and wound vacs
• Experience with medication administration and reconciliation
• Experience with feeding tubes, drain care, (trach care a plus)
• An ability to develop care plans and work with clinical teams to seek out solutions
• Wellsky (Kinnser) EHR

The following skills are preferred:

• One year of experience in the Home Health setting

Required credentials will include:

• A valid LVN license from the state of California
• Annual physical
• Proof of TB screening
• CPR or BLS certification
• Ability to pass a background and license check
• Valid driver’s license
• Proof of citizenship
• Proof of Covid vaccination with booster
