Oakdays is part of Project Homekey of Alameda County, a Permanent Supportive Housing program designed to provide individuals who have experienced homelessness. Oakdays provides permanent housing to medically vulnerable individuals who have an identified need for ongoing personal care or nursing intervention. Care is facilitated by enrollment in California benefit programs and provided by patient care teams in collaboration with Homekey clinical personnel. Oakdays aims to support residents through the care continuum and provide housing and quality healthcare with compassion and dignity.

Job Purpose
Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management.
Qualifications

  • Hold current license as Licensed Vocational Nurse through the California Board of Registered Nursing.
  • Current Basic Life Support Certification
  • Current two years of experiences as a Registered Nurse
  • Experience with homeless or incarcerated populations is highly desirable.

Tasks

  • Collaborate with housing staff to facilitate resident enrollment in medical benefits, connection with medical home, and other social services.
  • Collaborate with resident’s Primary Care and other professional disciplines to ensure effective care delivery and achievement of desired outcomes.
  • Ensure residents are provided with appropriate information to make informed choices related to treatment and that care reflects resident preferences.
  • Maintain communication with Homekey partner pharmacies, facilitate safe medication use on site in accordance with individual resident needs.
  • Ensure adherence to infection control standards on site among staff and residents.
  • In collaboration with housing agency, ensure daily wellness of residents.
  • Identify and respond to immunization needs of residents.
  • Plan and execute population health interventions among residents and staff that contribute to facility safety and health.
  • Ensure that clinical documentation is current, timely, and accurate.
  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Utilize and contribute to Alameda County data platforms such as the Community Health Record, Salesforce platform, and others.
  • Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation.
  • Facilitate post-hospital transition plan of care for OAKDAYS residents discharged from Emergency Department or inpatient hospital stays.
  • Plan and execute interventions to reduce number of ambulatory-sensitive Emergency Department visits.
  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.

Work Requirements

  • Physical Demands: Employees may frequently need to stoop, kneel, crouch, stand, walk, sit, use arms, hands and fingers. Employees will frequently need to communicate and listen.
