Position Summary:


Under the direct supervision of a Registered Nurse, administers nursing care to ill, injured, convalescent, or disabled patients, to include medication administration, treatments, and assistance with procedures, personal care, and activities of daily living. Ensures the health, comfort, and safety of patients. Consults and coordinates with health care team members to assess, plan, implement, and evaluate patient care plans. Follows multidisciplinary plan of care as indicated by patients’ progress. Participates in patient education on health maintenance and disease prevention. Monitors, records, and reports symptoms and changes in patients' conditions. Records patients' medical information and vital signs. Reviews and reports diagnostic test results to physicians and Registered Nurse. Monitors all aspects of patient care, including diet and physical activity. Prepares patients for and assists with, examinations and treatments. Communicates with physicians, patients, patients’ families, visitors, and co-workers. Behaviors demonstrate commitment to exceptional patient experiences through Service Excellence initiatives. Duties vary and increase in complexity as levels increase.


Knowledge, Skills, and Abilities:

Understands and routinely uses policies and procedures as reference for performance. Follows written plan of care. Has knowledge of the information and techniques needed to provide basic nursing care of human injuries, diseases, and deformities. Applies fundamental problem-solving and interpersonal communication skills. Demonstrates awareness of essential hospital standards and regulatory requirements. Actively participates in unit-based Performance Improvement projects. Meets NC BON educational requirements for Continuing Competence. Functions as part of a larger multidisciplinary team. Assists with provision of patient education. Identifies and appropriately addresses basic patient safety needs. Demonstrates experience-based knowledge; recognizes when plan of care needs to be modified in response to unachieved outcomes. Reviews and revises plan of care with Registered Nurse oversight. Applies clinical judgment and discretion based on clinical experience and knowledge of standards of practice. Implements moderately complex plan of care, integrates it through the whole continuum, and plans for next level with other health care providers (i.e., physician, case manager). Identifies patient and family learning needs and provides education under the direction of an RN; teaches CNAs and mentors other LPNs and new employees. Functions as part of a larger multidisciplinary team. Excellent problem-solving skills, excellent critical thinking and interpersonal communication skills. Good leadership skills and positive role model. May lead unit-based Performance Improvement projects and participates in departmental and/or organizational committees. Exceeds contact hour requirements of NC BON for Continuing Competence.


Required Education, Licenses, and Certifications:
  • Diploma or Certificate Program Practical Nursing
  • Posses current NC Practical Nurse license
  • Current BLS required



Other Information:

Speech and auditory acuity for communication of patient needs. Visual acuity for observation of patients. Much standing, walking, bending, and lifting. Position may involve skin, eye, mucous membrane, or parenteral contact with blood or other potentially infectious material.
