Job Title – LPN
Contract - 13 Weeks (Possible Extension)
Shift – Days - 8:30 am - 5:00 pm
Location – Lawton, OK 73505

SUBMITTAL REQUIREMENT:

· High school diploma or equivalent.

· Current Oklahoma LPN license required.

· BLS/ACLS(AHA): Required

· 1+ years LPN Long-term care experience is required.

· Two years of medical-surgical experience preferred.

Responsibilities:

· Participates and implements age specific care and interaction.

· Employee has attended departmental orientation (training) and annual inservice education for age specific training and implements in work area what was learned in age specific training.

· Obtains daily schedule of patients to be seen and verifies each appointment by calling patients.

· Ensures all patients scheduled will be seen.

· Patients are informed if their scheduled time is unable to be kept and an estimated time of arrival given, i.e., am or pm.

· Identifies common recurrent patient problems, symptoms, and behavioral changes.

· Documentation reflects these areas in the assessment completed on the nurses' notes.

· Involves and instructs the patient, family, and/or caregiver in the developing of the treatment plan.

· Instructions are documented on the patients’ nursing notes and signed by the nurse.

· Adds to the treatment plan and medication sheet when specific patient care is indicated.

· Administers prescribed treatments as permitted by state law.

· Documentation reflects physician’s orders and interventions received by the Licensed Nurse.

· Informs the physician and other personnel of changes in the patient's condition and needs.

· Prompt physician contact is documented with telephone orders implemented without error.

· Instructions to the patient, family, and/or caregiver are documented when completed.

· Documentation will reflect education and instructions given to patients/families. For example, brochures, handouts and flyers.

Pay: $30.00 - $34.00 per hour
