We are seeking qualified candidates interested in joining our Residential Group Home team at our Wilmington, Delaware location.

The Licensed Practical Nurse (LPN) as part of the Residential Group Home team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participant’s medication and provides psychiatric nursing services consistent with physician’s orders.

Some of the responsibilities include the following.

  • Deliver patient care utilizing the nursing process including assessment, planning of care, implementation, resident and family education and evaluation of care.
  • Instruct, counsel and provide support to assigned residents, focusing on client independence and individual needs.
  • Maintain an environment which encourages proactive decision making and growth for each resident.


Job Qualifications:


  • Nursing licensure LPN in the state of Delaware or Compact to include the state of Delaware.

  • Knowledge of mental health systems and supports.

  • Knowledge of substance abuse systems and supports.

  • Good communication skills both oral and written.

  • Ability to work independently and as part of a team.

  • Possess a valid driver’s license with an acceptable driving record.

  • Strong organizational skills.

  • Successful completion of the Child Abuse Registry and the Adult Abuse Registry background check with no adverse registry findings, and obtain service letters in accordance with 19Del Code Section 708 and 11 Del Code Sections 8563 and 8564.

Scope:

  • Frequent talking and listening.

  • Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.

  • Local travel required, including transporting participants.

  • May be required to report to work during emergencies, including inclement weather.

  • May require crisis intervention in emergency situations.

  • Scheduling flexibility and possible on-call work are necessary.
