PURPOSE OF POSITION:

To provide behavioral health services for the patients/families and support persons as appropriate in meeting their psycho social needs. To work in conjunction with the care team to ensure that efficient care with appropriate use of resources is provided in an effective time frame, striving for positive patient outcomes while meeting the MCHC mission.

JOB SPECIFIC RESPONSIBILITIES & EXPECTATIONS:

  • Provide therapeutic services to patients and their families.
  • Work in collaboration with multidisciplinary team members including external partners and assists in developing and fostering the team-based approach in the care model.
  • Demonstrates cultural competency and commitment to patients.
  • Fosters role as advocate via ongoing assessment of patient needs to promote positive outcomes.
  • Functions as reference and liaison for community resources and services available to patients and their families.
  • Provides education and information to the team regarding patients in their areas of responsibility as needed.
  • Communicates concerns as appropriate (Performance Improvement, Infection Control, Risk Management).
  • Works closely with coding and billing to ensure information documented to meet the requirements of the third party payers.
  • Identifies opportunities for inter and intra-departmental operational improvements.
  • Provides coverage for team members as needed.
  • Participates in regular meeting forums.
  • Addresses referrals from providers including referrals via warm hand-offs.
  • Participates in daily huddles to assess needs and coordinate services.
  • Maintains weekly scheduled appointments in our electronic records system- Allscripts.
  • Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner.
  • Define and build our SBIRT program.
  • Maintain case load of 15 to 20 patients weekly, and minimum of 5 patients a day.
  • Other duties as assigned.

QUALIFICATIONS:

  • Graduate of approved school with degree in social work or related field.
  • Current LISW licensure in the State of Ohio.
  • Strong preference for previous Federally Qualified Health Center (FQHC) experience.
  • One year experience in an outpatient setting preferred.
  • Refined communication skills, both written and oral.
  • Knowledge of managed care concepts in relation to financial aspects of patient care.
  • Knowledge of coding and the ability to apply correct codes to services provided.
  • Must be able to cope with the emotional and mental stress of the position and deadlines imposed on specific duties.
  • Refined organizational skills with well-developed style of diplomacy, assertiveness, and a tolerance for ambiguity.
  • Able to use critical thinking skills, problem solving abilities and an understanding of systems and change process.
  • Able to function independently, with autonomy, displaying personal integrity and always maintaining confidentiality.
  • Able to work with all levels of the multidisciplinary healthcare team.
  • Refined negotiating skills and the ability to deal with conflict resolution.
  • Functions in the role as patient/family advocate, intervening as needs indicate.
  • Dresses in professional manner.

GENERAL REQUIREMENTS:

  • Completes required annual TB testing, Confidentiality Statement, and Internet Access Statement.
  • Utilizes available educational resources to maintain and develop knowledge and skills related to field of practice.

Job Type: Full-time

Pay: $60,000.00 - $70,000.00 per year
