NELC is hiring a Full-Time Liaison Nurse to join our growing team! This nurse will cover the Cape Cod and Southern Massachusetts areas while accommodating patients being discharging from not only Cape Cod Hospital and South Shore Hospital but those who choose to seek care in Boston hospitals while living in the Southern Massachusetts communities.

Job Responsibilities*

*
Gathers and communicates to NELC’s Intake Department demographic, clinical and reimbursement information on patients referred to NELC.

  • Develops relationships and seeks opportunities to bring all potential home infusion referrals to NELC. This may include developing relationships within the hospital, among ambulatory care providers, physician offices and within the member hospital’s care providers (i.e. urgent care, SNF, rehab, etc).
  • Facilitates the discharge process by relaying orders and documentation necessary for discharge to the appropriate NELC departments and distributes to the appropriate member of the healthcare team for processing.
  • Coordinates NELC services with home health agencies.
  • Communicates effectively with pharmacy, reimbursement, materials management and delivery staff regarding new referrals and ongoing patient issues.
  • Effectively communicates patient requirements and changes in patient status to appropriate members of infusion and reimbursement teams as well as the HHA staff involved in meeting the patient’s home health needs.
  • Comprehensively documents all patient related information / conversations in patient’s clinical record.
  • Where appropriate, liaison will participate in daily patient care meetings and care conferences providing updated patient information as needed.
  • Communicates all changes in patient supply requirements to pharmacy/ operations team as necessary for referral coordination.
  • Utilizes clinical documentation tools to document referral communication including the electronic medical record and other supporting databases. Comprehensively documents all interactions with patients and members of the healthcare team in the patient record and/or electronic progress note.
  • Collaborates as an active member with all staff in the organization, particularly with the Clinical Liaisons, Teach Nurses, and Partnership Development staff to actively support account growth.
  • Acts as liaison between NELC and all appropriate departments at the hospital, ambulatory care providers, physician’s offices and within the member hospital’s care providers (i.e. urgent care, SNF, etc.) or the hospital’s system of care. This includes, but is not limited to, the medical staff, care coordination, case management, social services, pharmacy and nursing departments.
  • Assesses the needs and abilities of the patient and caregivers. Participates in developing a patient specific training plan and plan of care. Participates in patient training when appropriate.
  • Communicates results of insurance verification to referral source and/or patient. Assists NELC reimbursement department in exploring all possible sources or reimbursement.

Required Qualifications / Skills*

  • BS in Nursing preferred.
  • Associate degree or diploma in Nursing
  • Two (2) years intravenous therapy experience in hospital or homecare recommended
  • Current BLS Certification preferred.
  • Working knowledge of healthcare reimbursement or home infusion reimbursement preferred.
  • Prior experience in care coordination, home health or home infusion desired.
  • R.N. or L.P.N. license in good standing in the state(s) where providing care / services.
  • Valid driver’s license
  • Excellent driving record
  • Knowledge of HIPPA Privacy and Security Regulations preferred
  • Knowledge of The Joint Commission accreditation process and standards preferred
