Job Description Summary:
To maintain patient rights by educating patients, providing assistance to patients and their families in resolving issues or concerns. Responsible for welcome to Medicare and Medicaid annual exams.
  • Adheres to the code of professional nursing and maintaining current, thorough knowledge of nursing practices and procedures.
  • Thoroughly and accurately assesses initial and on-going data pertinent to patient care; communicating assessment to practitioner.
  • Maintains patient confidentiality and displays respect for patient dignity at all times.
  • Provide accurate, complete, and timely documentation of all nursing procedures in the medical record, utilizing appropriate reporting mechanisms, as required by NRMC standards and the law.
  • Provide appropriate information, counseling, and educational resources to allow the patient to participate in their care.
  • Administer routine immunizations, therapeutic injections and skin tests; maintain appropriate and accurate documentation of results in the medical record.
  • Participate in the performance of clinical procedures including but not limited to: blood pressure checks, documentation of medications, venipunctures, and routine treatments such as inhalation therapy, catheterization, dressing changes, within the scope of nursing practice and under the supervision of the provider.
  • Comply with all infection control and safety procedures in the disposal of any waste materials to include bio-hazardous waste.
  • Provide daily environmental cleaning and equipment quality control and maintenance. Order and maintain current inventory of medication (ensuring medication checks for outdated medications), supplies, and other materials as needed by the clinical staff. Ensures emergent medications and IV trays are properly stocked and maintained.
  • Participate in and obtain continuing education requirements and programs consistent with the level and scope of nursing practice. Assists with the orientation of new personnel. Remain in compliance with all NRMC policies and procedures as well as State and Federal regulations
  • Patient Care Coordination:
  • Reports to Community Health Improvement Manager
  • Coordinate communication between patients, family members, medical staff, administrative staff, or regulatory agencies.
  • Interview patients or their representatives to identify problems relating to care.
  • Maintain knowledge of community services and resources available to patients.
  • Assists with performance improvement efforts within the organization.
  • Development, implementation and maintenance of policies and procedures, standards, and programs that support patient Care Transitions.
Shift:
Day Shift - 10 Hours (United States of America)
Schedule Details:
Please know schedules and shifts are subject to change based on patient care and department/organizational needs.
4 10hr shifts per week. Scheduled day off to be determined upon hiring decision
