LCSW Counselors Needed!! – MUST be Licensed Clinical Social Worker (LCSW) - Spanish Speaking a PLUS +++

We offer a choice of options to work with clients in their residential home, nursing homes, assisted living buildings and/or a combination of all environments.

Essential Job Duties and Responsibilities:

• Provides on-site clinical counseling to clients (and families when needed).
• Conduct Psychosocial Assessment, Progress Notes and Psychotherapy Treatment Plan and mental health screenings for all clients.
• Participate in case conferences, as needed with Eaves and building's care team Develops, coordinates and implements discharge plans in collaboration with patient, patient's family and tx team.
• Weekly/Bi-Weekly discussion/updates with Director to give/get updates and discuss general happenings.

Qualifications Needed:

• Must be licensed in Illinois as an LCSW or Licensed Clinical Psychologist (Psy.D or Ph.D)
• Experience or interest in working with aging populations.
• Valid Driver’s License
• Experience in a Assisted Living, nursing home or Healthcare setting a plus+

Full-time, Part-Time and 1099 Contractor opportunities are available.

Salary range based on employment (FT, PT or FFS - 1099 Contractor)

Pay: $65,000.00 - $93,000.00 per year
