EQUIPMENT OPERATION

The job requires the ability to utilize a PC, calculator, multi-line telephone, and other related office equipment.

We need someone to work in office , Not Remotely.

Responsibilities The schedule is Monday-Friday 8:30am-5pm

Spanish speaking is a plus

Coordinates the patient’s and families care and services including those of the team, social worker, home health aide and physician

Responds necessary information to the Director of Nursing

Monitors the re-certification / re-authorization process

Ensures and documents appropriate use of the medications, durable medical equipment and supplies

Educates patients, families and other caregivers

Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Dietary, Speech and Pharmacy

Provides appropriate care plan and integrates care plans as needed

Participates in routine chart reviews

Attends regularly scheduled in-services and educational conferences.

QUALIFICATIONS

1. Must have a High school education. (Preferred College or university)

2. Must have a minimum of 1-year Home Health Care experience, (ability to use DSL Software)

3. Must possess typing and other clerical skills.

4. Fluent in English speaking and writing, Spanish is a plus

Pay: From $19.00 per hour

Experience:

• Medical terminology: 1 year (Preferred)
• Computer skills: 1 year (Preferred)
• Home health: 1 year (Required)
