Job Hours: 8:30 AM – 5:00 PM – Monday – Friday - 8 hours per day

View Point Health’s Community Support Services (CSS) is a client-centered, recovery oriented and community based service for individuals with persistent mental illness. CSS utilizes a holistic approach to provided skills building supports in the community.


  • The Housing Outreach Coordinator (HOC) will provide Housing Coordination Services to individuals involved in various Housing Programs. The initial goal is to assist adult individuals with securing and maintaining stable housing.
  • This position requires the ability to work in a fast paced setting, a passion to work with those experiencing homelessness or housing instability, knowledge and experience of providing housing related case management, and the ability to work closely and effectively with a team of professionals.
  • Reliable transportation and driving abilities is imperative.
  • The Housing Outreach Coordinator (HOC) will provide intensive housing support coordination & individual training for the adult homeless population.
  • The housing outreach coordination job responsibilities will include working closely with housing units/complexes, private landlords, community partners/agencies, and other View Point Health programs.
  • Occasional flexibility in scheduled hours is necessary to provide effective services throughout the day and into the evening hours.
  • On-call rotations may be a necessary function of the Coordinator’s role.
  • Services may be provided remotely, in the client’s residence, or their community.
  • This position also works closely & meets on a weekly basis with a team of professional staff to assess needs, develop treatment plans, and monitor client progress.

Job Description:

  • The HOC will identify those homeless individuals within Gwinnett County who are in need of housing supports and resources. Homeless individuals will be identified by local agencies, View Point Health programs, street outreach, and through our partnership with the Gwinnett County Jail and Police Department.
  • The HOC will be responsible for linking homeless individuals to housing supports/ resources as well as other resources identified a being needed such as mental health, case management, skills building, legal resources, and AOD supports & resources.
  • The HOC will also be responsible for but not limited to partaking in treatment team meetings, on call rotation, H.M.I.S coordinated entry, progress notes, developing partnerships, and making appropriate referrals. Reliable transportation and driving abilities are imperative.
  • Interview client, family members, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
  • Participate in community outreach to individuals experiencing homelessness or housing instability
  • Prepare accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.
  • Conduct a minimum of 1 home visit per week/per client within their community setting or within client’s current residence.
  • Assisting individuals with the application process for various housing subsidies/programs.
  • Assisting with identifying units for leasing and apartment complex partners
  • Provide care coordination to ensure stability of housing and independent living skills.
  • Placement of individuals into permanent housing
  • Supporting the individuals once placed in permanent housing
  • Assisting with management of the permanent housing units
  • Attending and participating in team meetings
  • Provide on-call services as necessary to manage crisis situations
  • Recognize the need for crisis intervention and notify supervisor/professional staff of escalated situations
  • Collect and report on data required for grants and other funding sources.
  • Develop and maintain a network of community contacts. Facilitate referrals and linkages to community services and communication among client’s service providers with VPH to assure a continuum of services.
  • Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources.
  • Meet or exceed Agency Productivity standards
  • Perform other duties as assigned.

Job Requirements/Competencies:

  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must be able to lift 20 pounds.
  • Must have the following technical skills – Word, Excel.
  • Must be able to stand or sit for long periods.
  • While performing the duties of this job, the employee is frequently required to sit, move about, hear and speak.
  • Must be able to stand, sit, and kneel for long periods of time.
  • Other duties as assigned

Qualifications:

  • Bachelor’s degree in Psychology, Counseling, Social Work, or other closely related field
  • Bachelor's degree in a related field from an accredited college or university AND Two years of related experience OR High School diploma AND Four years of related experience AND valid driver's license with clean motor vehicle report. Minimum of 2 years’ experience providing HUD-related housing services (HCV, TBRA, Rapid Rehousing, PSH, etc).
  • Must be 21 years old; pass criminal records background check, pre-employment physical health and drug screenings.

Preferred Qualifications:

  • Bachelor’s degree in Psychology, Counseling, Social Work, or other closely related field
  • One (1) year of experience specifically working with adults with severe and persistent mental illness and addictive disease
  • At least one (1) year of experience working within housing coordination
  • At least one (1) year of experience providing in-home/community services - Knowledge of community resources within metro counties
  • Exemplary verbal and written communication skills
  • Knowledge of community resources within Gwinnett County and surrounding areas
