YOUR DUTIES AT A GLANCE:
The purpose of this position is to provide Housing Support Services to Medicaid service consumers in need of necessary supports to obtain and reside in an in-home setting. Supports are tailored to the goal of maintaining a consumer’s personal health and welfare. Supports are offered through consultation with a consumer’s Medicaid Case Manager or in working directly with a consumer. Housing Support Services include but are not limited to conducting a tenant screening and housing assessment, developing an individualized housing support plan, developing a housing support crisis plan, identifying, and sharing resources, assisting with application processes and move details, offering education and training, coaching and providing assistance, and advocating. Develops and maintains knowledge of community resources for housing support services and shares information with Medicaid Case Manager, APD leadership, other ODHS staff, and community partners
WORKING CONDITIONS:
Working environment will be in office and in the community. This position will usually work a standard workweek. There may be some irregular hours due to consumer needs. Day travel will be regularly required within the service area. Travel may be in inclement weather. A valid driver’s license is required.
WHAT YOU NEED TO QUALIFY/MINIMUM QUALIFICATIONS (MQ’s):
Six months of experience providing services to persons who are elderly or disabled;
AND training and experience which shows knowledge of at least four (4) of the areas listed:
1) Medical and psycho-social problems of the elderly and disabled.
2) Normal and abnormal human development and behavior.
3) Appropriate intervention methods for the elderly and disabled.
4) Medical terminology and procedures.
5) Implications of illness or injury upon clients and families.
6) State and Federal rules about long-term care, medical services, and clients' rights.
7) Policies and procedures about agency programs.
8) Available community resources.
ESSENTIAL ATTRIBUTES:
  • Strong ability to communicate with diverse groups and individuals in areas such as, responding to inquiries, narrative reports and instructional materials with limited supervision.
  • Communication/listening skills in order to interpret and develop the ideas/needs of consumers and visualize the best approach to achieve the desired outcome.
  • Work positively as a member of a team.
  • Experience in working with consumers who are aged, physically disabled or relevantly equivalent.
  • Experience working with community partners
