The Family Case Management Coordinator is accountable directly to the Home Visiting Program Manager. Provides home visitation services and ensures Family Case Manager's activities meet the requirements of Healthy Families America and Growing Great Kids. Utilizes Healthy Families America approach to promote positive parent-child interactions and healthy childhood growth and development. This position is responsible for supervising two Family Case Managers and carrying a caseload of 7-8 families.

Highlighted Duties and Responsibilities:
  • Reviews initial Parent Survey assessment and intake forms and assigns to home visitors to ensure service delivery within 48 hours of receipt
  • Develops HFA Service Plans based on the Parent Survey in collaboration with the Home Visiting program manager for all families assigned to caseload.
  • Conducts quarterly shadow home visits with Family Case Managers.
  • Ensures staff are completing required assessments (PHQ-9 depression scale, Abusive Behavior Inventory [ABI], Ages and Stages Questionnaire, CHEERS Check-in, etc.) and establishing Family Goal Plans within established time frames outlined in the Home Visiting Policy Manual.
  • Meets weekly with the Home Visiting Program Manager for 90 minute reflective supervision
  • sessions, per HFA standards.
  • Conducts weekly 90-120 minute reflective supervision meetings with each Family Case Manager.
  • Meets with supervisees regularly and provides ongoing feedback and support, including opportunities for professional development.
  • Participates in monthly quality management activities as directed to ensure high quality, high impact services to families.
  • Makes referrals for health services to Community of Hope as needed.
Minimum Qualifications:
  • B.A. in Human Services related field (social work, early childhood education, family studies, psychology) or equivalent professional experience required.
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date.
  • Experience with providing public health case management and education (i.e., reproductive health, smoking cessation, child development, and parenting) to at risk populations preferably in the clients home setting strongly preferred
  • At least 1 year of supervisory experience required.
  • At least 2 years of experience in public health and/or human services field required.
  • Demonstrated cultural competence in communicating with underserved, diverse populations.
  • Strong written and verbal communication skills.
  • Ability to work with computers and electronic health records.
  • Knowledge of community resources or the ability to become knowledgeable required.
  • Ability to establish and maintain trusting relationships with families.
  • Strong interpersonal, organizational, analytical, problem solving and cross-cultural sensitivity skills required.
  • Ability to work flexible hours including weekends and evenings required.
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.
  • All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines. Medical or religious exemptions may be requested.
