Floral Home Care agency is looking to hire experienced Physical Therapists (PT) and Occupational Therapists (OT)

Responsibilities

• Identify and meet patients needs and goals.
• Develops a plan of care for the patient identifying short and long term goals and patient and caregiver involvement.
• Observes, records and reports to the physician and the nurse, the patient reaction to treatment and any changes in the patient condition.
• Familiar with stretching, soft tissue mobilizations, myofascial release, manual therapy and corrective exercises
• Consult and practice with other health professionals
• Maintains communication with physician and other personnel involved in the patient care, through written reports, attendance at meetings and confirmed telephone calls.
• Completes initial and ongoing assessments to determine patient level of functioning

Requirements:

* NYS PT/ OT License

We offer Great Pay with room for growth.

Pay: $55.00 - $65.00 per hour
