Summary: The Health Services Director, RN (Facility) (HSD) is a working manager who is responsible for oversight of the Health Services Department and staff. The HSD ensures that service provision is in compliance with Illinois Department of Public Health and CARF regulations.

Requirements:

  • Registered Nurse in the state of Illinois
  • Experience working in a psychiatric setting
  • Basic computer skills, including email, Word, and Excel
  • Managerial experience
  • Knowledge of SMHRF and CARF regulations
  • Training, as prescribed by Illinois Department of Human Services Division of Mental Health (DMH/DHS), prior to assuming the duties of this position (or within the last 5 years)
  • Criminal background is free of Disqualifying Offenses, as defined by the Illinois Department of Public Health (IDPH)

Essential Duties:

Regulatory

  • The HSD oversees the clients’ clinical record, making sure all paperwork that is pertinent to Health Services including:
  • Consents upon admission and annually or as needed
  • Immunizations
  • Evaluations
  • Physician Order Sheets
  • Medical Administration Record System (MARS)
  • TB testing
  • Orders given
  • Clinical nursing notes/documentation are thorough and reflect interventions related to clients care plan goals
  • Ensure client care plans are complete, reviewed at least quarterly and all annual nursing assessments are completed in a timely manner
  • Assessments completed as needed such as: Fall, Post-Fall and Pain Scale
  • The HSD attends monthly Safety Committee meetings and Quarterly Quality Assurance meetings to provide data related to the Health Services Department

Operations

  • Oversee various health related programs, infection control, activities of daily living (ADL) programs such as dental hygiene, hand washing, and others as services dictate
  • Reviews all incident reports for accuracy and to ensure they are complete
  • Qualifying incident reports, as defined by IDPH, are reported to IDPH within 24 hours
  • HSD completes a follow-up on all incidents within 7 days
  • Incident report follow-ups are faxed to IDPH, as per IDPH regulations
  • Reviews charting for thoroughness and accuracy
  • Provides training and in-services to all staff on health services topics
  • Works in tandem with the Executive Director and Clinical Director to screen admissions for appropriateness to the center
  • Reviews or designates someone to review new admissions and readmission of clients to ensure paperwork is complete and correct
  • HSD coordinates and tracks employee drug screens and physicals
  • Works closely with the pharmacy to ensure client medications are accurate, up-to-date and in stock
  • Supervises and oversees the Health Services Department including the nurses and CNA/Rehabilitative Services Associate (RSA)
  • Must attend scheduled clinical supervision
  • Other duties as assigned

Client Care

  • Ensures that client medical needs are met
  • Respond to crisis situations to assist in de-escalation and conflict resolution
  • Monitor client behavior and symptomology and ensure that proper interventions occur
  • Coordination of client care with client, psychiatrist, medical doctor, and other providers as it relates to physical health concerns
  • Ensure that health services staff are responding to client needs in a timely and respectful manner

Supervisory Responsibilities

  • Supervises and over-sees the Health Services Department including the nurses and CNA/RSA
  • Manages the schedule for all nursing shifts making sure the facility is fully staffed
  • If a shift is not covered and agency could not come in, the HSD is responsible for covering that shift
  • The HSD is on call 24/7 to assist Health Services staff in handling medical issues and at times psychiatric issues occurring during non-business hours
  • The HSD is responsible for communicating these issues and the resolutions to the Executive Director
  • The HSD provides clinical supervision to staff through trainings, in-services and regular meetings with the Health Services staff
  • The HSD is responsible for hiring qualified and competent health services staff
  • The HSD is responsible for reviewing performance, holding staff accountable and following the center’s guidelines for progressive disciplinary action

Work Environment

Lifting Requirements: Medium work - Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
