About the Program:

Sunrise Services is recruiting for a Health Care Coordinator (HCC) to perform health care case management for individuals with developmental disabilities, and those with chronic mental illness. The HCC locates healthcare providers and specialists as needed, attends medical appointments, provides advocacy and transportation for participants, and ensures documentation of all health care contacts during and in between appointments.

The successful candidate will have basic healthcare knowledge, experience working with adults and/or children with developmental disabilities and/or mental illness, basic computer and office skills, exercise sound judgment in challenging dynamic situations and be able to read, assimilate and act upon multifaceted information. We are looking for someone who is reliable, prompt and always a professional.

Duties & Responsibilities:

• Perform healthcare case management to client's enrolled in the supported living program
• Locate health and community services for clients
• Assist clients in attending appointments
• Advocate for clients during and in between appointments
• Manage overall patient care

Requirements:

• Must be at least 21 years old
• Must have a High School diploma or GED
• Must be able to qualify to drive a company vehicle
• Must be a CNA or LPN
• Vaccination or face masks may be required.

Schedule:

Full Time, Monday - Friday 8am - 4:30pm

Compensation:

• $22 - $27/hour depending on experience and with CNA
• $30.00 - $35.00/hour depending on experience and with LPN
• Wages are determined by applicable industry experience, education, and certifications.
