Horizon Adult Health Care is excited to announce an opportunity to be part of Horizon as a full time Health Care Associate/Van Driver. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Also, this position is Monday through Friday with no nights, no weekends, no work on Christmas or other company holidays.

Under the supervision of the Adult Day Center Director, the Health Care Associate will assist participants with activities such as personal care, recreation and meals, as well as being cross trained to transport participants in company vans.

Position Responsibilities:

  • Provide assistance to participants including personal care, recreational assistance, and meal services as needed.
  • Responsible for Horizon Adult Health Care passengers during transport, providing care in a manner which ensures participant satisfaction.
  • Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the PACE Center. Communicates these changes to dispatch.
  • Operates equipment safely and in compliance with vehicle licensing regulations.
  • Maintains and completes daily required records i.e., Pre- and Post-Trip Inspections, gasoline purchases.
  • Immediately reports, to Adult Day Center Director, any vehicle or cell phone operating problems, malfunction, or incidents out of the ordinary routine.
  • Transports participants to and from medical/specialist appointments, and activity outings, as requested by the Interdisciplinary Team via the plan of care.
  • Transports meals, durable medical equipment, pharmacy, and any other items as deemed necessary for participant.
  • Provides physical assistance to each participant when needed as he/she boards and debarks from the program’s vehicles.
  • Maintains the vehicle in a neat and clean condition.
  • Ensures vehicle equipment is maintained and operating within manufactures recommendations, replacing supplies as needed.
  • Communicates with Adult Day Center Director daily to be updated on participant conditions/ changes, plan of care changes, or Interdisciplinary Team requests.
  • Follows all Horizon Adult Health Care program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
  • Participates in and supports Quality Improvement Initiatives.
  • Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses.
  • Performs other duties as required or requested.

  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment
  • Supports, cooperates with and implements specific procedures and programs
  • Supports and participates in common teamwork
  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances
  • Assists participants and caregivers in filing grievances

Experience & Education:

  • Must have a valid driver’s license, proof of insurance and have means of transportation. Be at least 25 years of age, able to pass DOT physical.
  • High school diploma or equivalent preferred
  • Preferred to have: Have a minimum of one year’s experience in working with the frail elderly.

Pay: $16.50 per hour
