Purpose of Your Job Position:
The primary purpose of this position is to work as a part of a team, teaching and assisting individuals who have been diagnosed with an intellectual disability, sometimes coupled alongside other disorders and/or co-occurring mental illness. You will be coaching them through their day to day activities and challenges guided by each client’s individualized habilitation plan. The overall goal is for the client to obtain the highest level of independence and adaptability he/she is capable of achieving.

Major Duties and Responsibilities:
  • Provide assistance and training in all personal hygiene activities; bathing; oral care; dressing; meals; adaptive equipment trainings; clothing changes, occasional incontinence care; toilet training programs; mealtimes; shaving; hair care; nail care; laundry; etc.
  • Follow and become well versed in each client’s Behavior Intervention Plan (BIP); record behavior data; attend intervention training as required; correctly utilize intervention training; utilize positive reinforcement methods as instructed; provide communication training as needed; etc.
  • Provide assistance and training in household chores, i.e. sweeping, mopping, dishes, cooking,
  • Provide assistance and training in consumer skills such as money management, shopping, transportation.
  • Provide training on and supervise the use of safety skills at home and in the community and participate / conduct safety drills.
  • Follow and become well versed in each client’s Individual Habilitation Plan (IHP).
  • Conduct training on client goals as written in each IHP
  • Providing accurate and timely documentation on all ADLs, goals, and incidents of behavior.
  • Maintaining a habilitative environment through supervision/interaction
  • Provide recreational activities designed to maintain and expand individual skills.
  • Establishing and maintaining a safe, clean working environment

  • Providing accurate and timely communication to the Interdisciplinary Team (IDT) regarding all aspects of your clients.
  • Provide vocational training as directed by the IHP
  • Attend required staff training meetings along with other required meetings.
  • Transport clients to appointments and/or work
  • Transport, engage, and, supervise clients during community-based outings
  • Participate in Special Olympics to support your clients
Education

Must possess, as a minimum, a 10th grade education.

Experience

Must be a licensed Certified Nursing Assistant (CNA) or Developmentally Disabled Direct Care Aide (DDCA) in accordance with laws of this state.
