Position Overview

General supervisory level in the habilitation care occupation under general supervision of Habilitation Care Supervisor II, Emergency Respite Shelter Supervisor, Social Program Supervisor and/or any other appropriate supervisor requiring working knowledge of habilitation practices and ability to obtain first aid certification and complete in-service training covering individual problem-oriented treatment plans during the probationary period in order to supervise assigned staff (i.e., Habilitation Provider) and participate in development and modification of individual habilitation plans. Normal size staff (5-17).

Summary of Job Duties

Supervises Habilitation Providers and directs work activities to ensure active treatment (e.g., implementation of PIPs as written, consistent implementation of behavior programs, integration of services with recreation, OT, PT, and speech, active participation in programming groups, safety and well being of residents, administers first aid as needed, instructs habilitation providers and pool personnel on specific programs/techniques, monitors accuracy and timeliness of implementation of individual habilitative plans and all required documentation, assigns staff for clinics, outings, transportation/bus duty, reports all major unusual incidents.
Performs tasks of the functional area(s) supervised, reviews documentation regarding resident behavior response and progress as relates to Individual Habilitation Plan for care assigned resident, assists habilitation providers provide treatment to residents, monitors purchase and use of residents' personal belongings including clothing, ensures adequate and appropriate supplies for individual programmatic needs as well as programming groups, ensures effective communication with pertinent personnel, attends meetings as required.
Monitors daily activity schedules; monitors planned meals and meals schedules; monitors time sheets by use of Kronos Payroll System; signs leave forms, ensures the rights of residents are protected, and immediately reports and appropriately addresses violations; helps to maintain accurate and up-to-date files; helps to ensure ongoing family contact, informs the supervisor/manager/case manager of specialized problems involving the resident, family and/or guardian.

Minimum Qualifications and Requirements

Equivalent to high school diploma and 4 yrs. experience participating in development and/or carrying out habilitation/treatment plans for persons with disabilities or severe to profound intellectual disabilities including 1 yr. experience as team leader -OR- 3 college level business/management courses -OR- alternative, equivalent evidence of the Minimum Class Requirements. Must have and maintain valid driver's license with an acceptable driving record and show proof of insurance. (Other Evidences May Be Substituted)

Supplemental Information

Competency based training in IDD/DD resident care; CPR and First Aid Certification must be successfully completed during new employee orientation. Must maintain certifications.

Seasonal Employees: Benefits include Sick Leave accrued at 4.6 hours for every eighty 80 hours in active
pay status. Seasonal employees scheduled to work 40 hours during their period of employment will accrue vacation in the same manner as regular full-time employees and can use their vacation after completion of twenty-six (26) bi-weekly pay periods in active pay status. Part-time Seasonal employees (scheduled to work less than 40 hours) do not accrue vacation.

Intermittent and Temporary Employees: Long-Term Sick Leave accrual is based on hours worked.
