Northwest Journey provides a community-based treatment option for children & adolescents with mental health issues. We offer comprehensive intervention and preventative services aimed at preventing out of home placements and creating family stability and individual success. In addition, Northwest Journey provides therapeutic support services for children, adolescents, and families transitioning back to the community from out-of-home placements.

The Care Coordinator is a central force of both the internal and external treatment team. She/He/They obtains, provides and utilizes all clinical information about the client to develop the treatment plan, present the client’s status, and communicate about the treatment process. The Care Coordinator is the primary liaison between all parties involved with the client’s case. Care Coordinators will receive supervision from the Program Director and Licensed Mental Health Professional.

Essential Duties and Responsibilities:

  • Provide clients and parents/guardians and outside providers / schools as appropriate with information regarding the goals of the program, initial assessments, treatment planning and reviews, and the rights and responsibilities of clients and their families. Facilitate family participation in the program and treatment plan as appropriate.
  • Communicate with day treatment staff regarding clinical and educational needs of the client and facilitate the development of a working treatment plan that addresses those needs.
  • Implement the treatment plan as a member of the treatment team.
  • Effectively facilitate interagency meetings as a representative of the program and treatment team including providing and facilitating ongoing reviews of client progress and treatment planning, facilitating discharge and aftercare planning, and mediating, if necessary, any conflict between the client and/or his/her/their family and the program.
  • Complete necessary documentation as required by certification, funding source, and program standards.
  • Maintain an understanding of mental health concepts, psychotropic medications, and client symptomology which allows for identification of client needs, development and implementation of the treatment plan, and provision of quality mental health services.
  • Build treatment resource knowledge and network within the community while providing positive public relations. Serve as an advocate for the clients and their families with other agencies and programs to help them obtain necessary services and benefits from those agencies or programs.
  • Other duties as assigned by the Program Director or Day Treatment and Clinical Services Director/Deputy Director.
Requirements:

QUALIFICATIONS UNDER DHS 40:
Mental Health Support Workers shall have a minimum of one year of experience or formal education related to working with youth who have severe emotional disturbance or mental illness.
