POSITION SUMMARY
The Float Social Worker travels between facilities to support the social services department and provide exceptional customer service to the residents residing at the facility. This position is intended to assist facilities with the social service tasks in the absence of the facility social worker (during FMLA, vacations, open positions, etc.). The Float Social Worker identifies the psychosocial strengths and needs of each resident, assists in developing an individualized plan of care, collaborates with residents and their care team to help the resident attain or maintain their highest level of functioning. This must be done in accordance with current federal, state, and local standards, guidelines and regulations, and adhering to the company’s established policies and procedures.
Required Education, Skills and Experience
- Must recognize and understand the psychosocial and emotional needs of residents
- Strong communication and organization skills
- Ability and willingness to travel to and between different Monarch communities as designated.
- Flexibility – effectively manage multiple tasks and adapt to schedule changes to accommodate for coverage needs at multiple facilities
- Interpersonal skills - ability to have a good relationship with all personality types and inspire trust with residents, families and staff
- Comfortable working in ambiguous situations and with diverse populations
- Ability to thrive in an environment that is fast-paced and rapidly growing
- Must be able to perform essential functions of the position with or without reasonable accommodation
- Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services
- Bachelor’s Degree in Social Work, Psychology, Human Services or related field
- Experience in healthcare, skilled nursing facility or assisted living community
