POSITION SUMMARY

Position reports to the Branch Manager or Operational Leader and has responsibility to ensure the company is delivering the ultimate client care and caregiver experience. The Field Supervisor ensures quality control of client care and seeks to maintain the positive morale of caregivers servicing those clients.

II. POSITION REQUIREMENTS:

1. High school graduate or equivalent

2. CNA state certification preferred or credentialed as a state approved caregiver

3. Excellent customer service and interpersonal skills via telephone and in-person

4. Ability to prioritize and organize workload

5. Good time management skills

6. Highly flexible, able to accommodate changing needs of the client, caregiver, or organization, in addition to being able to attend on the spot potential client and new client start of care meet & greets

7. Ability to work well under pressure with minimal supervision

8. Basic to intermediate computer knowledge and skills

9. Comfortable learning new computer applications (As required by state, HHA exchange, Sandata, or NetSmart)

10. Attendance at continuing education programs per agency policy

11. Availability via telephone and for home visits if necessary, during Agency on call hours.

III. DUTIES AND RESPONSIBILITIES:

1. Ensures quality, and safe delivery of home health care services.

2. May conduct an initial meet and greet with a potential client.

3. Coordinates with scheduling to identify the right caregiver for a client based on information gathered during an initial meet and greet or intake.

4. Conducts a start of care visit with the client and caregiver to review the plan of care and ensure all required responsibilities are clearly understood by the caregiver. This includes personal care needs, medication reminders, nutritional needs, and transfers.

5. Evaluates and documents the client, family member(s) and home situation to determine what assistance will be available from family members in caring for the client.

6. Case manages the care by assessing the changing needs of the client, recommends additional supports needed, and implements required measures.

7. Serves as liaison for the family, referral source, and payor source. Communicates status changes as required according to company policy.

8. Conducts outreach in the community to support business development.

9. Provides guidance to Care Coordinators to aide in assigning appropriate caregivers to meet the client’s ongoing needs.

10. Available to assist in scheduling as needed.

11. Makes home visits to evaluate and ensure the client’s needs are being met according to company policy. Visit also includes a review of the plan of care, an environmental check, supervision of the caregiver, and soliciting client/family feedback.

12. Maintains a positive and motivated caregiver workforce by establishing a regular cadence of communication and outreach.

13. Responsible for caregiver relationship management to include training, coaching, and required disciplinary action. Properly documents and keeps the office staff aware of such actions.

14. Follows up with patients on interruption to ensure continuation of services once off interruption.

15. Provides exceptional customer service by resolving all issues timely and completing incident reporting according to company policy.

16. Completes and submits all required documentation accurately and appropriately and in a timely fashion.

17. Responsible for providing services for clients as needed and may include transports to medical appointments (with proper client waivers), shopping, delivery of Personal Protective Equipment, and interventions with sensitive family dynamics and/or client issues.

18. Participates in required daily and or weekly meetings as required.

19. Ensures professional development by attending continuing education as requested by the company.

20. Can support caregiver orientations and conducts annual caregiver evaluations as required.

21. Utilizes EMR system as required.

22. Consistently adheres to universal precautions and infection control guidelines.

23. Consistently complies with standards for attendance, absence notification, and punctuality.

24. Consistently demonstrates professionalism through appearance, performance, and communication.

25. Maintains confidentiality regarding all aspects of patients and/or employee information in compliance with HIPAA rules. Maintains compliance with applicable state and federal regulations, company policies/procedures and accreditation standards. Adheres to high ethical standards.

26. Available via telephone and/or provide home visits as required during agency on-call hours.

27. Performs other duties as assigned.

IV. OTHER NECESSARY SKILLS

  • Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing

V. SUPERVISION & MANAGEMENT RESPONSIBILITY:

Has supervisory responsibility for client care performed by caregiver staff
