General Description:

The Family Support Coordinator works directly with the Family Support Team to provide Family Support Services in the Greater Taunton/Attleboro area, participates in relevant training opportunities and reports weekly to the Director of Family Support.

General Qualifications:

Bachelor’s Degree in Social Work or related field (preferred) and two to five years of experience, in the field, working with individuals with developmental and intellectual disabilities and with families. Other qualifications include working knowledge of resources, benefits, and services; good communication and organization skills; ability to make sound clinical decisions; and a commitment to working as a team member.

Job Responsibilities and Expectations:

  • Protect the rights and dignities of individuals supported and extend those rights and dignities to family members and guardians.
  • Understand and implement through work with families the Principles of Family Support and having working knowledge of DDS’s guidelines for Family Support.
  • Work with families to encourage active family involvement and to identify families’ strengths; provide consultation to identify interests and needs for purposes of developing activities and service options; provide information regarding generic resources and services; and work with families to maximize their natural supports.
  • Provide the following contracted services:
  • Information and Referral (assistance to families in identifying resources and service options available in their local communities, as well as learning about other financial and/or state services for which families may be eligible, and provides guidance on how to navigate the service system)
  • Family Trainings (provide educational events and trainings on relevant topics of interest to families)
  • Family Networking and Support (provide opportunities for parent and sibling support groups, as well as parental involvement in organizing or leading activities, etc.)
  • Community Connections & Resources (foster community affiliations and connections to explore and maximize the use of natural and generic resources and other potential sources of funding and for purposes of promoting community inclusion)
  • Service Navigation (provide comprehensive and individualized information, guidance, and support to families to address their needs, connect them to potential resources, assist with problem-solving, and help them navigate the service system by providing either short-term or extended service navigation)
  • Administration of Flexible Funding as approved by funding source.
  • Maintain standards related to individuals’ and families’ privacy and HIPAA regulations including obtaining releases of information.
  • Collaborate with community resources and services, State Departments, disability specific services to determine how best to meet the needs of families/individuals.
  • Provide support to families during transition periods of time such as from early intervention to school or from child to adult service systems.
  • Provide short term direct support for individuals/families as directed by supervisor and assist if appropriate in transition to another DDS service such as SSQUAL or Agency with Choice.
  • Work with The Arc’s Directors of Human Resources and Family Support regarding families’ need for support/respite staff. Provide direction, support, supervision, and evaluation for staff and ensure compliance with needed certifications. Assist families in recruitment of family managed staff and provide information regarding tax responsibilities to these families.
  • Work with The Arc’s Family Support Advisory Council in implementing annual goals.
  • Represent The Arc of Bristol County, Inc. while in the community in a positive and professional manner; consistently communicating the agency’s philosophy and mission. This includes interactions with all stakeholders: individuals, family members/guardians, community partners, etc.
  • Adhere to the Direct Support Professional Code of Ethics.
  • Follow all agency and department specific policies and procedures.
  • Maintain the timeliness of submission and ensure accuracy of all required documentation.
  • Maintain accuracy and timeliness in the submission of bi-weekly time sheet including entry in ADP system and monthly mileage forms.
  • Provide assistance with emergency respite placements.
  • Perform other duties as assigned.
