Community of Hope is currently seeking a dedicated, full-time primary care Family Nurse Practitioner to join our team in providing excellent, quality focused patient care to our DC community. This position provides a patient panel that is about 20% pediatrics and 80% adult preventive and chronic disease care. Community of Hope has integrated dental and behavioral health care into our medical clinics to provide team-based care for the whole patient. As a member of the healthcare team, you will be able to partner with other providers to develop and support patient goals that will positively impact their health outcomes. This is an outpatient only position. The practice utilizes the eClincal Works EMR. This is a full-time position that will be split between our Marie Reed Health Center (NW) and our Conway Health and Resource Center (SW).

Highlighted Duties and Responsibilities:
  • Provides comprehensive, evidence based primary care;
  • Collaborates with the health care team on issues of patient care;
  • Signs off on all laboratory results, diagnostic imaging results, and referral reports in a timely fashion; sharing this information with patients and healthcare team as appropriate;
  • Documents all patient encounters, including face to face, telephone, and electronic communications, in the medical record in a timely fashion;
  • Works one Saturday per month and shares telephone-only call responsibilities (approx 1:9)
Minimum Qualifications:
  • Completion of a Family Nurse Practitioner program of study required;
  • National Certification as Family Nurse Practitioner;
  • Licensed to practice in the District of Columbia;
  • Current CPR certification (ACLS or BLS for the Healthcare Provider or equivalent) required;
  • Excellent verbal and written communication skills;
  • Ability to work in a team oriented environment;
  • Sensitivity to ethnic minority communities and commitment to work with underserved population;
  • One year professional experience in ambulatory health preferred;
  • New-to-practice candidates with strong medical nursing background are welcome to apply;
  • Computer proficiency, EMR experience preferred;
  • Bilingual in Spanish, French, and/or Amharic a plus.
  • Shares Saturday clinic coverage with other providers by completing at least one Saturday shift per month. Able to work evening and weekend hours as needed.
  • In relation to remote work versus on-site expectations, this position is classified as the following: On-site: A majority or all of the position is required to work on-site. Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in person learning for a specific amount of time after hire.
  • All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines. Medical or religious exemptions may be requested.
