The hours for the ED Navigator are 4 10 hours days Wednesday - Saturday. The hours are 10:00am-8:30pm.

POSITION SUMMARY

The ED Navigator provides superior quality, competitive value and outstanding service by providing exceptional support to the Quality team, Emergency Room and hospital staff, and patients. The ED Navigator assists patients with special needs and ensures that each health center meets these patients’ particular needs and requirements. ED Navigators focus on patients who lack a medical home. Responsibilities include: directing patients to different resources and information in the healthcare system, coordinating screenings and appointments, and encouraging patients.

GENERAL EDUCATION REQUIREMENTS

High School Diploma or General Equivalency/Educational Diploma (GED) with 2 years of experience in a medical office or community health center or Associate’s Degree in Healthcare or

Healthcare Administration with 1 year of experience in either a clerical role or healthcare environment preferred.

ADDITIONAL QUALIFICATIONS

  • Excellent verbal and written communication skills
  • Excellent collaboration skills required for community relationship building
  • Automated office and PC experience required including proficiency in Microsoft Office (Word and Excel)
  • Understands health office routines and community resources
  • Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components
  • Strong communication skills (oral and written) and negotiation skills
  • Experience with navigation of an Electronic Medical Records (EMR) and Practice Management System (PMS).
  • Health insurance knowledge preferred
  • Ability to speak and understand the English language

PRIMARY ACCOUNTABILITIES

  • Responsible for utilizing the Electronic Medical Record (EMR) and Practice Management System (PMS) to determine a patient’s eligibility for services
  • Maintaining communication with patients, families, and the health care providers to monitor patient satisfaction with their clinic experience
  • Ensuring that appropriate medical records are available at scheduled appointments
  • Coordinating appointments with providers to ensure timely delivery of diagnostic and treatment services
  • Arranging language translation or interpretation services
  • Perform follow-up interventions with patients shortly after they leave the hospital and facilitating linkages to follow-up services
  • Assist patients with making appointments with providers and address non-clinical barriers to receiving care, including issues with transportation, communication, and finances
  • Assist patients in completing the required forms
  • Help patients and families access a support system
  • Provide emotional support and related information
  • Assist health center staff by identifying patients who need follow-up care, assistance with reminder calls and making appointments
  • Assist staff with patient enrollment in health insurance plans or BHA services and programs leading to an increase in community residents with access to primary and preventive health services
  • Assist Resource Specialists in assigned tasks as directed
  • Serve as a liaison between Brevard Health Alliance (BHA) and the hospital
  • Perform other related duties as assigned
  • Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s Key Performance Indicators and will be evaluated on a monthly and quarterly basis.
