JOB DESCRIPTION


JOB TITLE: Pharmacy Director

DEPARTMENT: Pharmacy

SUPERVISED BY: CEO


SUMMARY:


Fills orders for drugs, monitors patient drug therapies and provides drug information; supervises and directs supportive personnel. Job is performed appropriately for the age population served, as defined in the department's scope of service.


Prepares and dispenses drug orders per physician request according to established policies, procedures and protocols.


Ensures safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures and protocols. Contributes to the quality and effective operation of the department.


EDUCATION, TRAINING AND EXPERIENCE:


  • Graduate from an accredited School of Pharmacy. Mandatory one (1) year experience as a practicing Pharmacist. Must be licensed to practice pharmacy in Louisiana.
  • Complete a minimum of 1.5 approved ACPE units (15 hours of C.E.) each year as a prerequisite for annual renewal of license.
  • Demonstrate good judgment in investigation and resolving problems.
  • Possess the ability to work quickly and accurately.
  • Must be able to work on multi tasks simultaneously.
  • Have an extensive knowledge of drugs, pharmacology, pathophysiology, and drug interactions.
  • Have the ability to use various types of pharmacy equipment and instruments.
  • Capability to explain medication information to patients as instructed by Louisiana Board of Pharmacy.
  • Understand the governmental rules and regulations that apply to the practice of pharmacy.
  • The employee must show excellent human relation skills and be able to communicate with a wide variety of educational and cultural backgrounds. Must be culturally sensitive.
  • Possess the ability to communicate effectively all levels within the centers and community.
  • Use correct grammatical English and write legibly in order to complete appropriate charting requirements and paper work. Mandatory ability to read, write and speak English in order to be able to counsel patients in accordance with OBRA 90.
  • Possess excellent math abilities for pharmaceutical calculations.
  • Present a professional image and the ability to maintain the highest level of quality patient and community relations in representing the foundation.

JOB RESPONSIBILITIES:


1.Perform all pharmacist functions as detailed in Louisiana Board of Pharmacy Laws and Regulations.

Evaluate appropriateness of drug therapy.


2.Verify accuracy, efficacy, and safety of presented prescriptions.


3.Perform comparative reviews of medications prescribed relative to patient's chart.


4.Perform prospective drug utilization reviews and notify practitioner of over or underutilization, therapeutic duplication, drug or disease contraindications, incorrect dose or duration of treatment, drug-allergy interactions, clinical misuse and abuse, etc.


5.Provide patient education as advised by the Louisiana Board of Pharmacy.


6.Provide more advanced patient education to patients with two or more risk factors for cardiovascular events.


7.Be responsible for manufacturer drug recall communications and returns.


8. Observe prescribing trends and assist in inventory management.


9.Develop and maintain effective communication with center staff and administration.


10. Supervise and be responsible for supportive personnel.


11. Remain current with developments and trends in the practice of pharmacy.


12. Maintain knowledge of evidence-based clinical practice guidelines as published by medical experts.


13. Be receptive and sensitive to patients with socioeconomic challenges.


14.Assist Director of Pharmacy in other pharmacies.

COVID-19

SWLA Center for Health Services (SWLA CHS) has a policy that requires all employees to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they receive an approved exemption based on medical condition, or sincerely held religious belief that prevents them from being vaccinated.

If you are not fully vaccinated, you will be required to have received at least one COVID-19 vaccination prior to your first day of employment and provide proof of your vaccination status on the Orientation Day of your employment. For the two COVID-19 shot vaccination regimen, you will be required to provide proof of your second COVID-19 vaccination within the first 45 days of your employment. In some roles, you may be required to provide proof of full vaccination before you can begin to actively work. Failure to provide timely proof of your COVID-19 vaccination status will result in the termination of your employment with SWLA CHS.

If you are unable to be fully vaccinated due to medical condition, or sincerely held religious belief, you will be required to apply for an exemption within the first 5 days of your employment in order to remain employed with SWLA CHS. As a part of this process, you will be required to provide information or documentation about the reason you cannot be vaccinated. You will be required to have an approved exemption before you can begin to actively work. If your request for an exemption is not approved, you will be required to be fully vaccinated or your employment may be terminated.
