Description:

Position Objective:

Responsible for operational leadership of the office of health promotion and wellness, including nutritional counseling, employee and community wellness initiatives, population health improvements and collaborations, and event/outreach oversight. Instrumental in tracking ongoing activities as part of Community Benefit Report process and recommending policy and programs that will enhance the health of the people Luminis Health serves. This position has community visibility and requires independent judgment and understanding of health education, particularly efforts to better help underserved populations and those who need health education services.

Essential Job Duties:

1. Provides day-to-day leadership of staff, including scheduling, performance management, and mentoring.

2. Monitors dashboards for opportunities to improve performance and create goals and metrics for ongoing improvement.

3. Plans, projects and markets preventive health services and programs in conjunction with client, payors, and providers. Oversees and expands the scope of all community outreach activities and where appropriate, brings services and programs to sites within the communities.

4. Works closely with service line/business development leaders as well as community partners in outreach programs that reach targeted segments of the culturally diverse community and creates an active link with our system of care.

5. Designs, develops, and implements systematic mechanisms to assess the expectations, concerns and needs of the communities; oversees community needs assessment process; creates annual community benefits report in concert with decision support, finance, and clinical partners.

6. Participates in the planning and implementation of outreach activities, including presentations, screenings, and health fairs which are designed to promote the health status of the community. Provides leadership for community education outreach (CEO) process.

7. Manages budget process and contracts with vendors. Coordinates expenditures for staffing, services, and supplies in a cost-effective manner, utilizing community resources and identifying opportunities for financial support, when possible.

8. Works toward expanding Energize wellness platform. Develops and implements programs to educate the community about personal responsibilities and accountabilities for health and encourages wellness and illness prevention services.

9. Communicates with Marketing and Business Development Team and informs appropriate agencies about financial needs of community outreach services and obtains outside funding.

10. In coordination with Physician Relations and other wellness partners, expands health education and wellness services to physicians’ office practice, the business community, and other client groups.

Additional Skills & Qualifications:

Educational/Experience Requirements:

  • Master's degree in a related field or BA/BS with related equivalent experience.
  • Highly motivated, dynamic individual with proven experience in medical and/or public health. Excellent written and verbal communication skills to interact effectively with diverse systems.
  • Possess intellectual curiosity, problem solving-abilities, and the ability to handle multiple projects with changing priorities. Experience in managing people and processes and use of leadership abilities to positively influence decisions.
  • Ten years of experience that includes healthcare, wellness, and health education program development and implementation. Three to five years of management experience in a health care setting that includes profit/loss budget accountability, contract negotiations and strategic planning.

Required License/Certifications:

  • Community Health Worker Certification preferred.
