This Direct Support Staff person will work with the Lead Staff person, the residents and their families to create and sustain a safe, healthy, supportive and fun home in this self-directed setting. They will support residents’ participation in meaningful daytime activity by supporting their transportation or other needs (lunch preparation, appropriate clothing selection, etc.).They will also assist the residents’ participation in community activities (such as Challenger Baseball, Artists Unlimited, Fairport Canal Days and Mighty Dunks Basketball).

Specific Duties Include:

• In collaboration with the individual served, their families and the Lead Staff position implement habilitation plans based on the individual’s preferences, interests and needs.
• Provide person centered support to all individuals served in areas including but not limited to daily living skills, domestic skills, social skills, volunteer opportunities, time management, personal care needs, and, when needed, parenting skills.
• Support individuals with maintaining a clean, safe, and orderly home. This includes light housekeeping, meal preparation and laundry.
• Maintain open communication and cooperative work effort with Lead Staff Position, other staff and families.
• Promote the emotional, physical and personal well-being of individuals served.
• Report and/or document routine and unusual events including completing daily logs in a concise, understandable, legible and timely manner.
• Obtain and maintain all required trainings and apply that information to support people served.
• Provide transportation in accordance with the individual’s plan of service and follow proper safety procedures in all transportation situations.
• Maintain flexibility in work schedule based on needs of individuals assigned.
• Ensure individuals are treated with dignity.
• Ensure regular communications with families as agreed. Suggest changes as needed.
• Provide regular feedback to families related to residents’ health, scheduling conflicts, happiness, requests for community activities or behavior concerns
• Responsible for ensuring the safety and supervision of residents. May be required to stay on-shift after hours should there be an emergency, weather hazard, or other unforeseen occurrence.

Working Conditions and Expected Hours of Work:

• Part time position, works in a self-directed residential home that is open 24 hours a day.
• Shifts are for Saturday evenings (4:00 pm - midnight), other evening shifts available.
• Some shifts during inclement weather will be required.
• Must be flexible and dependable.
• Hourly rate is $17.00. Some benefits are available through the Fiscal Intermediary, Heritage Christian Services.

QUALIFICATIONS:

Education: High School Diploma or equivalent.
Experience: One year practical experience working with individuals with developmental disabilities preferred.
Additional Requirements:

• Possess a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
• Continuous use of a reliable, registered and insured vehicle.
• Clear background checks
• Maintain required certifications including, but not limited to First Aid/CPR
